摘　　要：目的探讨重症急性胰腺炎伴腹腔感染患者临床易感因素,为临床防治提供参考。方法回顾性分析医院2010年5月-2015年4月60例重症急性胰腺炎患者临床病历资料,根据患者是否感染分为观察组(感染)和对照组(非感染),各30例,分析两组间各项指标的差异性,同时对相关指标行logistic回归分析,找出相关影响因素。结果经分析发现,器官功能衰竭数(OR=3.597)、肠功能障碍(>5d)、腹腔积液(OR=2.975)、血钙(OR=2.121)为重症急性胰腺炎伴腹腔感染易感因素。结论血钙、器官功能衰竭数、腹腔积液和肠功能障碍为重症急性胰腺炎伴腹腔感染的易感因素,临床应加强对早期重症急性胰腺炎患者感染因素的管理及治疗,减少腹腔感染率。OBJECTIVE To explore the predisposing factors for severe acute pancreatitis patients complicated with abdominal infections so as to provide guidance for clinical prevention and treatment.METHODS The clinical data of60 patients with severe acute pancreatitis who were treated in hospitals from May 2010 to Apr 2015 were retrospectively analyzed,then the enrolled patients were divided into the observation group（the infection group）and the control group（the non-infection group）according to the status of infection,with 30 cases in each group.The indexes were observed and compared between the two groups,the logistic regression analysis of the related indexes was performed so as to find out the related influencing factors.RESULTS The analysis revealed that the predisposing factors for the severe acute pancreatitis complicated with abdominal infections included the number of organ failure（OR=3.597）,intestinal dysfunction（more than 5days）,peritoneal effusions（OR=2.975）,and blood calcium（OR=2.121）.CONCLUSIONThe blood calcium,number of organ failure,peritoneal effusions,and intestinal dysfunction are the predisposing factors for the severe acute pancreatitis complicated with abdominal infections.It is necessary for the hospital to strengthen the management of the factors for the infections and the treatment of the patients with severe acute pancreatitis in early stage so as to reduce the incidence of abdominal infections.
