血清VEGF在急性脑梗死病人脑微出血中的预测作用

摘　　要：目的探讨血清血管内皮生长因子（VEGF）对急性脑梗死病人脑微出血（CMBs）的预测作用。方法纳入58例急性脑梗死合并脑微出血（脑微出血组）病人,82例急性脑梗死不合并脑微出血（无脑微出血组）病人。检查血糖、血脂四项（三酰甘油、胆固醇、高密度脂蛋白、低密度脂蛋白）、血小板计数、血同型半胱氨酸,并采用VEGF ELISA制剂盒检测血清VEGF。结果单因素分析显示,年龄、高血压、血同型半胱氨酸及血清VEGF水平与脑梗死病人CMBs密切相关。VEGF水平与CMBs数目呈明显线性正相关（r=0.853,P〈0.05）。校正OCSP分型、年龄、高血压病、脑梗死OCSP分型、血同型半胱氨酸等干扰因素后,Logistic逐步回归分析显示,VEGF是脑微出血的独立危险因素（OR=2.33,95%CI：1.233~2.771,P=0.010）。结论血清VEGF增高是急性脑梗死病人脑微出血的独立危险因素,VEGF水平与CMBs严重程度相关。Objectlve To investigate the predictive role of serum vascular endothelial growth factor （VEGF） on acute cerebral infarction（ACI） patients with cerebral microbleeds.Methods Fiftyeight ACI patients with cerebral microbleeds and 82 ACI patients with out cerebral microbleeds were included in this study.Blood sugar, blood lipid （triglyceride, cholesterol, high density lipoprotein, low density lipoprotein）, blood platelet count, serum homocysteine and serum VEGF were checked.Results Univariate analysis showed that age, hypertension, serum homocysteine and serum VEGF level in patients with cerebral infarction was closely related to CMBs. The VEGF level was correlated with cerebral infarction OCSP subtypes （ P 〈0.05）.There was significant positive correlation between the VEGF level and the number of CMBs.Adjustment with OCSP subtype, age, hypertension and other factors, logistic regression analysis showed that VEGF was the independent risk factors of cerebral microbleeds （ r = 2.33,95% CI, 1.233 to 2.771, P = 0.010）.Conclusion The increase of serum VEGF is an independent risk factor for CMBs in patients with ACI, and the level of VEGF is correlated with the severity of CMBs.
