摘  要:目的探讨双源CT低千伏和自动管电流技术对恶性肿瘤患者的价值。方法收集2013年10月~2014年6月行CT随访的93例恶性肿瘤患者,将患者随机分为A组(100 k V自动管电流组)、B组(100k V组固定电流组)、C组(120 k V自动管电流组)和D组(120 k V固定电流组,标准条件)行胸腹盆增强检查,计算4组每次CT检查的CTDLvol、DLP和ED,比较它们在各组间的差异。测量4组图像的SNR和CNR,并比较它们在各组间的差异,肉眼主观评估4组的解剖结构和病灶图像质量并进行评分。结果 A、B、C和D组CTDLvol、DLP、ED总体比较差异均有统计学意义(分别为F=1114.203,P〈0.001;F=106.174,P〈0.001;F=106.163,P〈0.001),经两两比较,CTDLvol在各组之间差异均有统计学意义(P〈0.05),而DLP和ED,除B组与D组外,各组之间差异均有统计学意义(P〈0.05)。A和C组、C组和D组SNR有差异有统计学意义(P〈0.05),4组CNR比较差异无统计学意义(F=1.242,P=0.299),4组图像质量主观评分差异无统计学意义(F=1.107,P=0.351)。结论恶性肿瘤患者行胸腹盆部双源CT增强扫描随访,应用自动管电流技术和低千伏(100 k V)技术,在图像质量满足临床诊断的同时,辐射剂量显著下降。Objective To discuss the value of dual source CT with automatic tube current and low kVP technique in the follow-up of patients with malignancy. Methods From Oct,2013 to June, 2014, chest, abdominal and pelvic enhanced-CT were performed in 93 patients with malignancy. Patients were divided into 4 groups randomly: group A ( 100 kV automatic tube current), group B ( 100 kVP fixed tube current), group C ( 120 kV automatic tube current), group D ( 120 kV fixed tube current, standard protocol) .The radiation dose of each CT follow-up was compared among the 4 groups. The SNR, CNR and subjective images quality assessed visually were also compared. Results Among 4 groups, the CTDLvol, DLP, ED were significantly different (F = 1114.203, P 〈 0.001; F = 106.174, P 〈 0.001; F = 106.163, P 〈 0.001, respectively). The CTDLvol was significantly different between any two groups (P 〈 0.05). For DLP and ED, except between B and D group, there were significantly different between any other two groups (P 〈 0.05). The SNR showed significant difference between group A and group C, group C and group D (P 〈 0.05, respectively). Among the four groups, the CNR had no significant difference (F = 1.242, P = 0.299), the subjective quality scores also showed no difference (F = 1.107, P = 0.351 ). Conclusion The application of automatic tube current and low voltage ( 100 kV) technique in dual source CT reduced the radiation dose significantly, while maintaining acceptable images quality in enhanced, CT follow-up of chest, abdominal and pelvic in patients with malignancy.
