眼内异物摘出时间对眼内炎发生的影响

摘　　要：目的探讨眼外伤后眼内异物摘出时间对外伤性感染性眼内炎（以下简称眼内炎）发生的影响。方法回顾性分析2009年1月至2014年8月于吉林大学第二医院治疗的370例（373眼）的临床资料，按照眼内异物摘出时间统计眼内炎发生率，以#检验进行统计学分析。结果总体眼内炎的发生率为14．75％，眼外伤后≤24h摘出组的眼内炎发生率为10．45％，〉24h摘出组的眼内炎发生率为19．77％，两组差异有统计学意义（X2=6．404，P=0．011）。当≤24h无法摘出异物时，2d～2周内摘出组眼内炎发生率低于3周～3个月内摘出组，二者之间差异有统计学意义（X2=4．446，P=0．035）。结论≤24h摘出异物比〉24h摘出眼内炎的发生率低。当424h无法摘出异物时，在采取相应措施下，2d～2周内摘出异物亦可降低眼内炎的发生风险。Objective To investigate the effect of the time of the intraocular foreign body （IOFB） extraction on the occurrence of endopbthalmitis. Methods The medical records of 370 eyes of 370 patients with IOFB treated in the Second Hospital of Jilin University from Jan. 2009 to Aug. 2014 were reviewed. The incidence rate of endophthalmitis was calculated on the time of the [OFB extraction, using the X2 test to identify the relationship between each group. Results The endophthalmitis incidence was 14.75%. The incidence of endophthalmitis in the group IOFB extraction less than 24 hours was 10. 45% , and the incidence in the group [OFB extraction more than 24 hours was 19.77%. The incidence in ≤24 hours group had significant difference compared with that in more than 24 hours group （X2 = 6. 404, P = 0.011 ）. The incidence of endophthalmitis in the group IOFB extraction in 2 days-2 weeks was significantly lower than that in the group IOFB extraction in 3 weeks-3 months （X2 = 4. 446, P = 0. 035 ）. Conclusion The incidence of endophthalmitis in the group IOFB extraction ≤24 hours is lower than that in the group [OFB extraction 〉 24 hours. When the IOFB can＇ t be extracted in less than 24 hours, the extraction in 2 days-2 weeks can also reduce the risk of endophthalmitis after the repair of the wound and use of antibiotics.
