摘 要:目的探讨甲状腺乳头状癌组织中增殖抗原-67(Ki-67)的表达水平及其与临床病理因素之间的关系。方法完整收集甲状腺临床病理资料共计545例;其中甲状腺乳头状癌275例,甲状腺良性肿瘤270例,使用免疫组织化学SP法检测甲状腺癌中Ki-67的表达情况,并对临床病理结果进行对比分析。结果甲状腺乳头状癌、甲状腺良性肿瘤Ki-67阳性表达率分别为91.27%(251/275)、44.07%(119/270),癌灶与良性肿瘤相比,Ki-67表达明显增高(χ~2=220.28,P=0.00);多灶性甲状腺癌与单病灶Ki-67表达阳性率分别为(96.03%和71.83%),多灶性甲状腺乳头状癌Ki-67表达水平明显增高(χ~2=45.05,P=0.00);淋巴结转移者和未转移者Ki-67表达阳性率分别为81.20%(108/133)、47.18%(67/142),其中有淋巴结转移者Ki-67表达水平明显增高(χ~2=36.90,P=0.00),CD56阳性与阴性患者Ki-67表达阳性率分别为76.09%(70/92)、59.44%(107/180),其中CD56阳性组Ki67表达水平明显增高(χ~2=13.05,P=0.01)。结论 Ki-67蛋白在甲状腺癌中表达明显增高,其阳性表达与多灶性甲状腺癌、淋巴结转移及CD56阳性表达有关,提示Ki-67阳性表达对甲状腺癌侵袭性和淋巴结转移有一定预测意义。Objective To investigate the expression of Proliferating Antigen-67( Ki-67) in papillary thyroid carcinoma and its relationship with clinical pathological factors. Methods Clinical pathological data of 545 cases were collected,including 275 cases of thyroid carcinoma,270 cases of benign tumors of the thyroid gland. The expression of Ki-67 in thyroid carcinoma was detected by immunohistochemical SP,and the clinical pathological results were compared and analyzed. Results Ki-67 positive expression rates in papillary thyroid carcinoma and benign thyroid tumor were 91. 27%(251 /275),44. 07%(119 /270),respectively. Compared with benign tumor,the expression of Ki-67 in cancer group increased significantly( χ~2= 220. 28,P = 0. 00); Ki-67 expression positive rates in multifocal thyroid carcinoma group and single lesion group were 96. 03 % and 71. 83 %,respectively. And the expression level of Ki-67 in focal papillary thyroid carcinoma group increased significantly( χ~2= 45. 05,P = 0. 00); The positive expression rate of Ki-67 in lymph node metastasis group and non metastasis were 81. 20 %( 108 /133),47. 18 %(67 /142),respectively. And the expression level of Ki-67 in lymph node metastasis group increased significantly(χ~2= 36. 90,P = 0. 00). The positive expression rates of Ki-67 in CD56 positive and CD56 negative group were76. 09 %( 70 /92),59. 44 %( 107 /180),respectively. The expression level of Ki67 in CD56 positive group was significantly higher( χ~2= 13. 05,P = 0. 01). Conclusion The expression of Ki-67 protein in thyroid cancer was significantly increased. What 's more,the positive expression of Ki-67 was related to the multiple foci of thyroid cancer,lymph node metastasis and CD56 positive expression,which suggests that the positive expression of Ki-67 has certain significance in the prediction of thyroid cancer invasion and lymph node metastasis.
