胃癌患者血清IGFBP3和Neuropilin-1的表达水平及其临床意义

摘　　要：目的探讨胃癌患者血清胰岛素样生长因子结合蛋白3(IGFBP3)和神经纤毛蛋白-1(Neuropilin-1)水平及其与临床病理参数间的相关性。方法选取胃癌患者98例,同期收集体检者50例为健康对照组。检测2组研究对象血清IGFBP3和Neuropilin-1水平;分析胃癌组患者血清IGFBP3和Neuropilin-1水平与胃癌临床病理参数间的相关性;比较随访期间存活组与死亡组患者间血清IGFBP3和Neuropilin-1表达。结果与对照组比较,胃癌组患者血清IGFBP3水平明显减少而Neuropilin-1水平明显升高(P<0.01)。血清IGFBP3和Neuropilin-1的表达与胃癌患者的年龄、性别、大小、肿瘤位置无显著性相关(P>0.05),而与TNM分期、分化程度以及淋巴结转移呈显著性相关(P<0.01)。所有患者1年期随访结果:33例死亡,65例存活;死亡组患者入院时血清IGFBP3水平明显低于存活组,而Neuropilin-1表达明显高于存活组,比较差异有统计学意义(P<0.01)。结论胃癌患者血清IGFBP3水平明显降低而Neuropilin-1的水平明显增高,与胃癌的恶性程度关系密切,也可能是临床评价胃癌患者预后的重要指标。Objective To investigate serum levels of IGFBP3 and Neuropilin-1 of gastric cancer and its correlation with clinicopathological parameter. Methods 98 patients with gastric cancer were analyzed, and 50 healthy cases were the control group. Serum levels of IGFBP3 and Neuropilin-1 in both groups were detected. Correlation of serum levels of IGFBP3 and Neuro- pilin-1 with clinicopathological parameter of gastric cancer was analyzed. Serum levels of IGFBP3 and Neuropilin-1 between sur- vival group and death group were compared. Results Compared with the control group, serum level of IGFBP3 of gastric cancer obviously decreased and Neuropilin-1 remarkably increased（ P 〈 0.01 ）. Serum levels of IGFBP3 and Neuropilin-1 had no signifi- cant correlation with age, sex, size, and tumor location（ P 〉 0.05 ） , and had significant correlation with TNM stage, differentiation, and lymph node metastasis （ P 〈 O. 01 ）. After 1 -year follow up, there had 33 death and 65 survival, serum level of IGFBP3 in death group was remarkably lower than survival group, while Neuropilin-1 was obvioualy higher, there had statistical difference （P 〈 O. 01 ）. Conclusion Serum level of IGFBP3 of gastric cancer obviously decreased and Neuropilin-1 remarkably increased, and are closely related with malignancy grade of gastric cancer, and could be one of important indicators of gastric cancer.
