上斜肌减弱术矫正A型斜视效果分析

摘　　要：目的探讨上斜肌延长、后徙、断腱三种不同方法矫正上斜肌亢进的A型斜视效果分析。方法回顾性研究于2011年1月至2014年3月在沈阳爱尔眼视光医院诊治的上斜肌亢进的A型斜视患者共62例，其中行上斜肌延长20例，上斜肌后徙32例，上斜肌断腱10例。观察术前、术后第一眼位斜视度，上下转25。斜视度，上斜肌亢进程度，以及比较三组术前、术后第一眼位垂直斜视度差值及A征差值。结果各组内术前、术后垂直斜视度及A征差异均有统计学意义（P〈0．05）。各组术前、术后垂直斜视度差值在三组间的比较（F=1．142，P=0．326），术前、术后A征差值比较均无统计学意义（F=0．653，P=0．524）。结论上斜肌延长、后徙、断腱都能矫正上斜肌亢进的A型斜视。Objective To investigate superior oblique weakening including extension, recession and tenectomy correcting A-pattern strabismus with superior oblique overaction. Methods Sixty-two A-pattern strabismus patients with superior oblique overaction were studied from 2011 to 2014, including superior oblique tendon-expander 20 patients, tenectomy 10 patients and recession 32 patients. Postoperative followed up 6 months. Preoperative and postoperative strabismus degree in primary eye position were observed, strabismus degree of superduction and infraduction 25 degree were examined by synoptophore. The difference of vertical strabismus degree and A pattern between preoperative and postoperative were compared. Results The vertical strabismus and A pattern degree difference of three groups between preoperative and postoperative had statistical significance （P 〈0.05）. The vertical strabismus and A pattern degree difference of interclass had no statistical significance. Conclusions Superior oblique muscle extension, recession, tenectomy all can be corrected A-pattern strabismus with superior oblique overaction.
