乌司他丁对食管癌术后炎症的治疗效果

摘　　要：目的：评价乌司他丁（UTI）对食管癌术后炎症的影响。方法：100例食管癌手术患者均分为治疗组和对照组,治疗组在手术前30 min给予UTI 100 000单位静脉滴注,并于术后当天~第3天,再次给予持续静脉泵入UTI,对照组于同时点给予等量生理盐水;所有患者于术前、术后第1天及术后第7天检测血清中性粒细胞弹性蛋白酶活性（NE）、白细胞介素-6（IL-6）及IL-8的浓度,于术后第1天、第3天检测气管肺泡灌洗液（BALF）中的NE浓度。结果：两组患者术前血清IL-6、IL-8及NE比较,差异无统计学意义（P〉0.05）;与术前比较,治疗后两组患者血清NE浓度升高,第1天高于第7天（P〈0.01）;治疗后,治疗组同时点血清IL-6、IL-8水平升高的幅度小于对照组（P〈0.05）;治疗后,BALF中的NE浓度第1天高于第3天,且治疗组低于对照组（P〈0.05）。结论：UTI可降低食管癌患者手术后血清和BALF中炎症因子水平、抑制过度炎性创伤反应。Objective： To evaluate the effect of ulinastatin on inflammatory reaction after resection of esophageal carcinoma. Methods： 100 esophageal carcinoma patients were randomly divided into two groups. In the treatment group, ulinastatin 100 000 Unit was intravenously infused 30 min before oper- ation, and 200 000 U/d. continuous infusion after operation for 3 days. In the control group, normal saline followed the same protocols. Serum neutrophil elastase（NE）, interlukin （IL）-6, IL-8 and neu- trophil elastase of bronchoalveolar lavage fluid were respectively compared before, 1 d and 7 d after treatment; NE concentration level was detected 1 d and 3 d after operation. Results： Comparing NE with serum IL-6 and IL-8 before operation in both groups was not statistically significant （ P 〉 0.05 ）. Comparing with before operation, NE concentration level increased in both groups, 1st day was higher than 7th day （P 〈0.01 ） ; after treatment, increase level of serum IL-8 and IL-6 in treatment group at same time point was lower than control group （P 〈 0.05 ） ; NE of bronchoalveolar lavage fluid in treat- ment group, 1st day was higher than 3rd day, was significantly smaller than in control group （P 〈 0. 05）. Conclusion： The use of ulinastatin can reduce the inflammatory factors, inhibit inflammatory response.
