高压氧早期介入结合言语治疗对脑外伤运动性失语症的改善作用

摘　　要：目的探讨高压氧早期介入结合言语治疗对脑外伤致运动性失语患者语言功能的改善作用。方法选取2014年6月～2015年12月就诊于解放军第113医院脑外伤致运动性失语症患者40例，随机分为实验组和对照组，每组20例。实验组患者入院后待生命体征平稳，能够脱离呼吸机或移动时开始高压氧治疗，同时给予言语治疗。对照组患者入院后待生命体征平稳、意识清醒后，给予言语治疗。冶疗4周后，比较患者言语功能（复述、说、朗读）评分和失语症严重程度（The boston diagnostic aphasia examination, BDAE分级）情况，判定临床疗效。结果两组患者治疗效果均较冶疗前有所改善，实验组BDAE分级中2～3级及4～5级的人数较对照组显著增加（P〈0．05），在汉语标准测评中，复述、说、朗读能力得分均较对照组明显提高（P〈0．05）。结论高压氧早期介入结合言语治疗对脑外伤致运动性失语患者语言功能障碍的治疗效果优于单纯给予言语治疗组，因此是一种有效治疗脑外伤后语言功能障碍的康复策略。Objective To explore the effect of early hyperbaric oxygen combined with speech therapy on the improvement of language problems in patients with motor aphasia after traumatic brain injury. Methods 40 cases with motor aphasia after traumatic brain injury were enrolled in the People＇s liberation army l l3hospital from June 2014 to December 2015,which were divided into control group and experiment group with 20 cases in each group randomly. After the vital signs of the patients were stable, and the patients can be moved or without the help of breathing machine. The experimental group were accepted the early hyperbaric oxygen treatment, while they are also treated with speech therapy. Results The evaluation of boston diagnostic aphasia examination （BDAE） and the criteria for Chinese in both groups all had been improved. In experimental group, the number of patients whose BDAE aphasia grade were at level 2-3 and 4-5 were increased significantly than that of control group （P〈0.05）, In chinese standard evaluation, the patients in experimental group achieve higher scores than the patients in control group in repeating, saying and reading ability （P〈0.05）. Conclusion Early intervention of hyperbaric oxygen combined with speech therapy was superior to only taking speech therapy in treatment for language dysfunction after brain injury, it was an effective rehabilitate strategies for language damage after traumatic brain injury.
