摘 要:目的探讨血清糖类抗原125(CA125)、人附睾分泌蛋白4(HE4)及基质金属蛋白酶-9(MMP-9)三者联合检测在卵巢癌早期诊断中的价值。方法选取2014年2月至2016年2月间我院收治的60例卵巢癌患者作为观察组,并选取我院同一时期收治的45例良性卵巢肿瘤患者作为卵巢良性肿瘤组,以及45例健康志愿者作为健康对照组。采用电化学发光法(ECLIA)检测三组的血清CA125水平,并采用酶联免疫法(ELISA)检测其血清HE4及MMP-9水平,并对比三组上述指标的检测结果。结果观察组血清CA125、HE4及MMP-9水平均显著高于卵巢良性肿瘤组与健康对照组,差异有统计学意义(P<0.05);观察组中不同分期患者的上述血清标志物水平组间比较差异有统计学意义(P<0.05);且观察组上述各标志物检测的阳性率均明显高于其他两组,差异有统计学意义(P<0.05);并且在卵巢癌的诊断中,血清CA125、HE4及MMP-9三者联合检测的灵敏度为93.3%,明显优于各指标单独检测的灵敏度63.3%、83.3%、76.7%,同时三者联合检测的特异度为96.7%明显优于各指标单独检测的特异度74.4%、91.1%、81.1%,差异均有统计学意义(P<0.05)。结论单独检测患者血清CA125、HE4及MMP-9水平在卵巢癌的早期诊断中具有一定的价值,而三指标联合检测明显提高了检测的灵敏度及特异度,在卵巢癌早期诊断中的价值更高。Objective To investigate the value of combined detection of serum carbohydrate antigen 125 (CA125), human epididymis secretory protein 4 (HE4) and matrix metalloproteinase -9 (MMP-9) in the early diagnosis of ovarian cancer. Methods We enrolled 60 cases of ovarian cancer patients admitted in our department between February 2014 to February 2016 as the observation group, and selected 45 cases of benign ovarian tumor patients as the disease control group, and 45 cases of healthy volunteers as a healthy control group. The serum CA125 levels of all three groups were detected by electrochemical luminescence (ECLIA), and the serum levels of HE4 and MMP-9 were detected by enzyme linked immunosorbent assay (ELISA). The results of three indexes were compared among the three groups. Results The serum levels of CA125, HE4 and MMP-9 in observation group were significantly higher than those of the control group and the healthy control group, the differences were statistically significant ( P 〈 0.05 ). The serum marker levels in observation group at different stages were statistically different ( P 〈 0.05 ). Each marker' s positive rate in the observed group was significantly higher than that in two control groups, the differences were statistically significant (P 〈 0.05). In the diagnosis of ovarian cancer, the sensitivity of combined detection of serum CA125, HFA and MMP-9 was 93.3% which was higher than each marker detected individually, 63.3%, 83.3%, 96.7%, 76.7% for CA125, HE4 and MMP-9 respectively.
