消化道肿瘤淋巴结转移灶P53、bcl-2、survivin与化疗药敏性的关系研究

摘　　要：目的研究消化道肿瘤淋巴结转移灶P53、bcl-2、survivin与化疗药敏性的关系。方法选择62例消化道恶性肿瘤患者的原发灶及转移淋巴结的肿瘤组织,进行四氮唑盐(MTT)法原代细胞培养体外化疗药敏性实验,对肿瘤细胞对顺铂(DDP)、长春新碱(VCR)、紫杉醇(PTX)、甲氨蝶吟(MTX)4种常用化疗药物的体外化疗药敏性进行检测,并采用免疫组织化学法(SP)对原发灶及转移灶中的P53、bcl-2、survivin的表达进行检测。结果 P53与bcl-2在原发灶和转移灶间的表达呈显著相关性(P<0.05),而survivin无显著相关性(P>0.05)。DDP、VCR、MTX对转移灶的肿瘤细胞抑制率显著低于原发灶(P<0.05),而PTX在原发灶与转移灶对肿瘤细胞的抑制情况无显著差异(P>0.05);原发灶中的化疗药物抑制率均与P53、bcl-2、survivin不相关(P>0.05);而在淋巴结转移灶中,P53与DDP对肿瘤细胞的抑制率呈负相关(P<0.05),survivin及bcl-2均与PTX抑制率呈负相关(P<0.05)。结论消化道肿瘤淋巴结转移灶中P53、bcl-2、survivin的表达情况及化疗药敏性均体现出异质性,术后化疗应尽量避免化疗靶目标的偏差,以提升癌症综合治疗的效果。Objective To explore the relationship between expression of P53, bcl-2, survivin and chemosensitivity in lymph node metastases of gastrointestinal tumors. Methods The tumor tissue and metastasis lymph nodes of 62 patients with gastrointestinal tumors were collected, and the tumor cells chemosensitivity to DDP, VCR, PTX, MTX were detected by MTT assay, meanwhile the expression of P53, bcl-2 and survivin in primary tumors and lymph node metastases were detected by immunohistochemically. Results The expression of P53 and bcl-2 in primary tumors and lymph node metastases were significant correlation （P〈0.05）,while survivin were not （P〉0.05）. The inhibition of tumor cells of DDP, VCR, MTX in lymph node metastases were significantly lower than those in primary tumors（P〈0.05）, while PTX were not（P〉0.05）. There were no correlation between expression of P53, bcl-2, survivin and inhibition rate of tumor cells （P〉0.05）, but in lymph node metastases,the expression of P53 was negative correlation with inhibition rate of tumor cells for DDP inhibition（P〈0.05）, and both of survivin and bcl-2 were negative correlation for PTX（P〈0.05）. Conclusion The expression of P53, bcl-2, and survivin chemosensitivity in lymph node metastases of gastrointestinal tumors are different from primary tumors, so it is necessary to avoid the deviation of chemotherapy target postoperative as far as possible to improve the comprehensive curative effect.
