血清心肌酶变化在精神科临床观察中的意义

摘　　要：目的探讨血清心肌酶变化在精神科的临床意义。方法对2014年精神科新入院716例患者进行血清心肌酶(CK、CK-MB)水平监测。其中36例(5.02%)患者心肌酶水平明显异常,实际入组观察34例,同时选取心肌酶水平正常34例患者进行对照研究。对研究组患者的心肌酶水平进行动态观察,并对两组患者采用阳性与阴性症状量表(PANSS)评定精神症状并进行比较。结果研究组治疗前血清CK、CK-MB与治疗后1周比较差异有统计学意义(P<0.05),治疗后2周与入院时比较有显著改变,并有统计学意义(P<0.01)。研究组PANSS总分、激活性、攻击性等因子在治疗后1周与入院时比较差异有统计学意义(P<0.05),治疗后2周与入院时比较有显著改变并有统计学意义(P<0.01)。结论监测患者血清心肌酶水平有助于判断病情严重程度及评估抗精神病对心肌影响程度。Objective To investigate the clinical significance of changes in serum myocardial enzymes in the psychiatric department. Methods Serum myocardial enzyme （CK, CK-MB） levels of 716 newly admitted psychiatric patients in 2004were monitored.Myocardial enzyme levels of 36 cases （5.02%） observed abnormal. 34 patients were observed, and 34 cases with normal myocardial enzyme level were selected for comparison. Myocardial enzyme levels of these two groups were assessed and compared by using the Positive and Negative Syndrome Scale （PANSS）. Results There was statistically significant（p 〈0.05） between serum CK and CK-MB after one week treatment in study group, and there were significant changes（p 〈0.01） after two weeks treatment. There were significant changes among PANSS total score, activating, aggressive and other factors after treatment. Conclusion Monitoring serum myocardial enzyme levels can help determine the severity of patient＇s condition and assess the impact of anti-psychotic to the myocardium.
