摘　　要：目的研究干细胞关键转录因子Oct4在人膀胱癌组织中的表达及对5 637细胞生物学的影响,为临床治疗提供依据。方法选取2015年1月至2016年1月74例膀胱癌组织、32例癌旁组织以及24例正常膀胱癌组织病理标本。采用免疫组化法检测标本中的Oct4表达;体外培养膀胱肿瘤细胞,采用MTT法检测siRNA敲除Oct4后对与细胞增殖作用的影响;采用细胞划痕及Transwell试验测定细胞迁移、侵袭能力的影响;分析干细胞关键转录因子Oct4在人膀胱癌组织中的表达及对5 637细胞生物学的影响。结果人膀胱癌组织中12例Oct4蛋白检测阴性,62例Oct4蛋白检测阳性,阳性率为83.78%,显著高于膀胱癌组织的21.87%(P<0.05);正常组织中Oct4蛋白阳性率为16.67%,三种不同细胞组织Oct4蛋白阳性率比较差异具有统计学意义(P<0.05)。膀胱癌组织Oct4蛋白表达在不同临床分期中无统计学意义(P>0.05);膀胱癌组织Oct4蛋白表达在不同病理分级、淋巴结转移中差异具有统计学意义(P<0.05);病理分级越高、淋巴结转移,Oct4蛋白表达越高;免疫组化结果显示:膀胱癌组织中Oct4蛋白表达阳性率为83.78%,膀胱癌旁组织Oct4蛋白表达阳性率为21.87%;正常组织Oct4蛋白表达阳性率为16.67%。三种不同的细胞进行划痕试验2 h后进行观察:结果显示:Oct4-siRNA转染后细胞迁移显著减弱,而其他细胞大量迁入划痕区域。结论 Oct4的表达与人膀胱癌的分级、淋巴结转移关系密切,但是不同临床分期差异不具有统计学意义;Oct4高表达在膀胱癌的发生、发生中发挥重要作用,能抑制膀胱癌细胞系Oct4基因表达,抑制肿瘤细胞增殖,细胞细胞凋亡,具有较高的临床应用价值。Objective To study the expression of key transcription factor Oct4 in human bladder cancer tissue and its effect on 5 637 W biology, and to provide evidence for clinical treatment. Methods Pathological specimens of 74 cases of bladder carcinoma tissues, 32 cases of paracarcinoma tissues and 24 cases of normal bladder cancer tissue were selected From January 2015 to 2016 January. Immunohistochemical method was performed to detect samples of Oct4 expression ; Bladder tumor cells were cultured in vitro, MTT assay was performed for the detection of siRNA knockdown effect on cell proliferation after Oct4 and cell migration and invasion were determined by cell scratch and Transwell test. Expression of stem cells key transcription factor Oct4 in bladder carcinoma and its influence on 5 637 cell biology were analyzed. Results The 12 cases of bladder cancer Oct4 protein were negative, 62 cases of Oct4 protein detection positive rate was 83. 78% , significantly higher than 21. 87% of bladder cancer tissues （ P 〈0.05） . Normal tissues Oct4 protein positive rate was 16. 67% , the difference of three different tissue protein Oct4 - positive rate was statistically significant （ P 〈 0 .05 ） . Bladder carcinoma Oct4 protein expression didnt related with clinical stage （ P 〉 0 .05 ）; The difference of bladder carcinoma Oct4 protein expression and tumor grade, lymph node metastasis was statistically significant, respectively （ P 〈0.05） ; the higher the pathological grade and the appearance of lymph node metastasis, the higher the Oct4 protein expression; immunohisto- chemistry results showed bladder cancer tissues Oct4 protein expression rate was 83.78% , adjacent bladder carcinoma Oct4 protein expression rate was 21. 87%.
