摘 要:目的探讨Twist2及P16在正常子宫颈组织、子宫颈低级别上皮内病变(LSIL)、高级别上皮内病变(HSIL)、子宫颈早期浸润癌、子宫颈中晚期癌中的表达及作用。方法采用免疫组化En Vision法,检测正常子宫颈组织20例、LSIL 20例、HSIL 20例、子宫颈早期浸润癌40例、子宫颈中晚期癌20例中Twist2、P16的表达,分析其临床意义。结果在子宫颈病变中,随着病变的逐渐加重,Twist2和P16阳性率均逐渐升高。其中,正常子宫颈组织分别与LSIL、HSIL、子宫颈早期浸润癌癌、子宫颈中晚期癌各组比较,Twist2和P16阳性表达差异存在统计学意义(P<0.05);在子宫颈早期浸润癌中,Twist2及P16在ⅠA1期和ⅠA2期阳性表达差异无统计学意义(P>0.05)。Twist2和P16在早期子宫颈癌的表达,经Spearman分析,两者存在高度相关性(r=0.972,P<0.05)。<50岁组Twist2的阳性表达率明显高于≥50岁组,两组间差异有统计学意义(P<0.05),P16阳性表达差异无统计学意义(P>0.05)。结论在子宫颈病变过程中,Twist2和P16表达率逐渐升高,Twist2对子宫颈早期浸润癌的病理诊断有指导价值。Objective To investigate the significance of Twist2 and P16 expression in normal cervical tissue, lower cervical intraepithelial lesion (Low - grade squamous intraepithelial lesions, LSIL) , high - grade intraepithelial lesion ( high - grade squamous intraepithelial lesions, HSIL) , early infiltrating carcinoma of the cervix and middle - late cervical carcinoma. Methods Paraffin - embedded samples of normal cervical tissue (20 cases) , LSIL (20 cases) , HSIL (20 cases) , early infiltrating carcinoma of the cervix (40 cases) and middle - late cervical carcinoma (20 cases) were the research objects. The expression of Twist2 and P16 were detected by immunohistochemical method in the cervical tissues, and the clinical significance was analyzed. Results With the disease aggravating, the positive expression rate of Twist2 and P16 are increased. Twist2 and P16 expression in normal cervical, LSIL, HSIL and cervical invasive cancer tissue have significant difference ( P 〈0. 0 5) . In early cervical invasive cancer, Twist2 and P16 positive expression have no statistically significant difference in I Al and I A2 term ( P 〉 0. 0 5 ) . There was high correlation in the expression of Twist2 and P16 in early cervical cancer via analysis of Spearman ( r =0.972, P 〈 0 .0 5 ) . Before age of 50 years and after age of 50 years, Twist2 expression have significant difference ( P 〈 0 .0 5 ) , however, the P16 expression have no significant difference (P 〉0.05). Conclusion In the development of the cervical lesions, Twist2 and P16 expression rate increased, Twist2 is important to the pathologic diagnosis of early cervical invasive cancer.
