经颅多普勒彩超对大脑中动脉狭窄患者脑血管储备力的评估及其与神经功能的关系

摘　　要：目的研究经颅多普勒彩超对大脑中动脉狭窄患者脑血管储备力的评估价值及其与神经功能的关系。方法选择轻度狭窄、中度狭窄和重度狭窄的患者各10例作为狭窄组,选择健康体检者30例作为对照组,采用经颅彩色多普勒超声联合屏气试验检测脑血管储备力,采用酶联免疫吸附法检测各组血清中内皮素-1(ET-1)、神经肽Y(NPY)、血管内皮生长因子(VEGF)、血管生成素(Ang-1)、S100β、神经元特异性烯醇化酶(NSE)含量。结果 (1)狭窄组患者大脑中动脉的屏气指数(BHI)、平均血流速度(Vm)上升率均低于对照组;且大脑中动脉的狭窄程度越重,BHI、Vm上升率越低;(2)狭窄组患者的血清ET-1、NPY、VEGF、Ang-1、S100β、NSE含量高于对照组;且大脑中动脉的狭窄程度越重,血清ET-1、NPY、VEGF、Ang-1、S100β、NSE含量越高;(3)BHI、Vm上升率与血清中NSE和S-100β的含量呈负相关。结论大脑中动脉狭窄患者的脑血管储备能力减弱;经颅多普勒超声测定的脑血管储备能力能够准确评估大脑中动脉狭窄患者的神经功能。Objective To study the evaluation value of cerebrovascular reserve capacity by transcranial color Doppler in patients with middle cerebral artery stenosis and its correlation with nerve function. Methods Patients with mild stenosis, moderate stenosis and severe stenosis, 10 cases for each type, were enrolled in stenosis group; 30 cases of healthy people were enrolled in control group. Cerebrovascular reserve capacity were detected by transcranial color Doppler combined with breath holding test. Serum ET - 1, NPY, VEGF, Ang - 1, S100p and NSE contents were detected by enzyme linked immunosorbent assay. Results ①Both BHI and Vm increase rate of stenosis group were lower than those of con-trol group. The more severe the degree of middle cerebral artery stenosis was, the lower the BHI and Vm increase rate were.②serum ET - 1, NPY, VEGF, Ang - 1 , S100p and NSE levels of stenosis group were higher than those of control group. The more severe the degree of middle cerebral artery stenosis was, the higher the serum ET - 1, NPY, VEGF, Ang - 1, S100p and NSE levels were. ③BHI and Vm increase rate were negatively correlated with serum NSE and S - 100 p contents. Conclusion Evaluation shows that cerebrovascular reserve capacity in patients with middle cerebral artery stenosis weakens; cerebrovascular reserve capacity detected by transcranial color Doppler ultrasound can accurately assess neurological function in patients with middle cerebral artery stenosis.
