摘  要:目的探讨肿瘤干细胞标志物CD44^+/CD24^(-/low)和乙醛脱氢酶1(ALDH1)在浸润性乳癌组织中的表达及其与乳癌分子亚型和预后的关系。方法制备组织芯片,使用罗氏Ventana Benchmark XT autostainer仪器,采用免疫组化法检测259例乳癌手术切除标本中CD44、CD24和ALDH1的表达。结果乳癌病例中CD44^+/CD24^(-/low)的阳性表达率为37.1%,其在乳癌各分子亚型中的表达差异具有统计学意义(χ~2=17.84,P<0.01),在Her-2型及三阴性乳癌(TNBC)中阳性表达率较高。乳癌病例中ALDH1的阳性表达率为22.8%,其在乳癌各分子亚型中的阳性表达率差异具有统计学意义(χ~2=28.11,P<0.01),在TNBC中阳性表达率较高。乳癌病例中CD44^+/CD24^(-/low)与ALDH1+的联合表达率为5.8%,其在乳癌各分子亚型中的阳性表达率差异具有统计学意义(χ~2=18.07,P<0.01)。CD44^+/CD24^(-/low)与ALDH1的表达无明显相关性(r=-0.11,P>0.05)。CD44^+/CD24^(-/low)阳性表达的乳癌病人5年无病生存率(DFS)为55.2%,较阴性表达病人显著降低(χ~2=10.34,P<0.01);ALDH1阳性表达的乳癌病人5年DFS为61.0%,较阴性表达病人显著降低(χ~2=12.28,P<0.01)。结论乳癌干细胞标志物CD44^+/CD24^(-/low)和ALDH1在乳癌组织中的表达不高,在乳癌各分子亚型中的表达有差异,联合检测这些标志物有可能预测病人预后。Objective To explore the expressions of CD44~+/CD24^(-/low) and Aldehyde dehydrogenase-1(ALDH1)in invasive breast cancer tissue and their relation with molecular subtypes of the cancer and its prognosis Methods Preparing tissue array,using roche Ventana Benchmark XT autostainer instrument and applying immunohistochemistry method,the expressions of CD44~+/CD24^(-/low) and ALDH1 in 259surgical resection specimens of the cancer were detected. Results In breast cancer,the expression rate of CD44~+/CD24^(-/low) was 37.1%,the difference of expression in different subtypes was statistically significant(χ~2=17.84,P〈0.01),its higher expression was noted in Her-2 and Triple negative breast cancer(TNBC).The positive rate of ALDH1 was 22.8%,its difference in different subtypes was significant(χ~2=28.11,P〈0.01),and a higher expression was noted in TNBC.The combined expression rate of CD44~+/CD24^(-/low) and ALDH1 was 5.8%,which was different in different molecular subtypes(χ~2=18.07,P〈0.01).There was no significant correlation between the expression of CD44~+/CD24^(-/low) and ALDH1(r=-0.11,P〈0.05).The 5-year disease-free survival(DFS)rate was 55.2%in the patients with positive CD44~+/CD24^(-/low) expression,which was lower than in the patients with negative expression(χ~2=10.34,P〈0.01).The 5-year DFS rate in patients with ALDH1 positive was 61.0%,much lower than in those with ALDH1 negative(χ~2=12.28,P〈0.01). Conclusion The expressions of breast cancer stem cell markers CD44~+/CD24^(-/low) and ALDH1 are not high,there are differences in the expression of various subtypes of breast cancer.A combined detection of these markers may be used to predict prognosis of the patients.
