摘 要:目的探讨胰腺癌患者外周血中内皮细胞选择素(E-selectin)、血小板选择蛋白(P-selection)、细胞间黏附分子-1(ICAM)、血管细胞间黏附分子-1(VCAM)、活化白细胞细胞黏附分子(ALCAM)的检测水平及其临床意义。方法选择2014年4月至2015年8月期间我院普外科收治的40例胰腺癌患者(胰腺癌组)作为研究对象,其中淋巴结浸润14例,伴肝转移11例;临床分期I^II期24例,III^IV期16例。选择同期胰腺炎患者40例(胰腺炎组)、健康体检者40例(对照组)进行比较。采用ELISA法检测ICAM、VCAM、Eselectin、P-selection、ALCAM水平。结果胰腺癌组ICAM、VCAM、E-selectin、ALCAM水平高于胰腺炎组和对照组,胰腺炎组高于对照组,差异均有统计学意义(P<0.05);胰腺癌组P-selection水平低于胰腺炎组和对照组,胰腺炎组低于对照组,差异均有统计学意义(P<0.05)。III^IV期胰腺癌患者ICAM、VCAM、Eselectin、ALCAM水平高于I^II期者,差异有统计学意义(P<0.05);III^IV期胰腺癌患者P-selection水平低于I^II期者,差异有统计学意义(P<0.05)。胰腺癌伴淋巴结浸润患者ICAM、VCAM、E-selectin、ALCAM水平高于不伴淋巴结浸润者,差异有统计学意义(P<0.05);胰腺癌伴淋巴结浸润患者P-selection水平低于不伴淋巴结浸润者,差异有统计学意义(P<0.05)。胰腺癌伴肝转移患者ICAM、VCAM、E-selectin、ALCAM水平高于不伴肝转移者,差异有统计学意义(P<0.05);胰腺癌伴肝转移患者P-selection水平低于不伴肝转移者,差异有统计学意义(P<0.05)。结论胰腺癌患者存在血管内皮功能的紊乱,且血管内皮功能紊乱与胰腺癌的临床病理有关。Objective To study the levels of ICAM, VCAM, E-selectin P-selection, ALCAM in peripheral blood and their clinical significance in patients with pancreatic cancer. Methods Forty patients with pancreatic cancer admitted in our hospital between Apr. 2014 and Aug. 2015 were selected as pancreatic cancer group. Of the 40 cases, 14 with lymph node invasion, 11 with liver metastases; 24 cases with clinical stage I-II, 16 with III-IV. 40 pancreatitis patients(pancreatitis group) and 40 healthy physical examination objects(control group)were selected at the same time. The levels of ICAM, VCAM, E-selectin, P-selection, ALCAM were detected by ELISA method. Results ICAM, VCAM, E-selectin, ALCAM levels in pancreatic cancer group were higher than those in pancreatitis group and control group, and those in pancreatitis group were higher than in control group(all P〈0.05); P-selection levels in the pancreatic cancer group were lower than those in pancreatitis group and control group, and those in pancreatitis group were lower than in control group(all P〈0.05). For pancreatic cancer patients with stage III-IV, the ICAM, VCAM, E-selectin, ALCAM levels were higher than in those patients with stage I-II(P〈0.05); and the P-selection levels in patients with stage III-IV pancreatic were lower than in those patients with I-II(P〈0.05). ICAM, VCAM, E-selectin, ALCAM levels in pancreatic cancer patients with lymph node invasion were higher than those in patients without lymph node invasion(P〈0.05); and the Pselection levels in patients with lymph node invasion were lower than those in patients without lymph node invasion(P〈0.05). ICAM, VCAM, E-selectin, ALCAM levels in pancreatic cancer patients with liver metastasis were higher than those in patients without liver metastasis(P〈0.05); and the P-selection levels in pancreatic cancer patients with liver metastasis were lower than those in patients without liver metastasis(P〈0.05).
