原发性乳腺恶性淋巴瘤九例诊治体会

摘　　要：目的总结原发性乳腺恶性淋巴瘤临床特征及诊疗方法。方法回顾性分析9例原发性乳腺恶性淋巴瘤患者的临床资料。结果 9例患者中,6例患者术前行粗针穿刺活检,仅2例术前诊断为乳腺恶性淋巴瘤;9例患者均行手术治疗,其中6例行乳腺癌改良根治术,1例行单纯乳房切除术,2例因术前诊断为乳腺恶性淋巴瘤仅行肿瘤切除术;9例患者术后均给予环磷酰胺+阿霉素+长春新碱+泼尼松(CHOP)方案化疗,2例患者后续行局部放疗;9例患者初治后均获得完全缓解,随访期间死亡2例,分别死于脑转移和骨髓转移。结论手术切除是乳腺恶性淋巴瘤主要治疗手段,术后辅助化疗及局部放疗可提高治疗效果。Objective To analyze the clinical data of primary breast lymphoma patients and summarize the clinical features and treatment methods. Methods The clinical data of nine patients with primary breast lymphoma in our hospital were retrospectively analyzed. Results In the six patients with preoperative coarse needle biopsy, only two cases were diagnosed as primary breast lymphoma preopera- tively;all the nine patients received surgical treatment, including six cases of modified radical mastecto- mies, one case of simple mastectomy, and two cases of tumorectomies for malignant lymphoma; all the nine patients received chemotherapy in accordance with CHOP plan and two patients received local radiothera- py subsequently;all the patients obtained complete remission after initial treatment, but two patients died of brain and bone marrow metastasis respectively during the foliow-up period. Conclusion Primary breast lymphoma is a rare malignant tumor with limited and specific clinical presentations. Surgical resection is still the main method for the treatment of primary breast lymphoma, and adjuvant chemotherapy and local radiotherapy may improve the therapeutic effects.
