摘 要:目的:通过探讨CD147和MMP-2在人脑胶质瘤中的表达及临床意义,为临床治疗提供参考。方法:选择2010年1月至2015年5月本院手术切除并经病理诊断确诊的脑胶质瘤标本70例作为观察组,根据WHO分型,将其分为低级别组31例和高级别组39例。另选取20例脑外伤患者并作内减压切除的标本作为对照组。采用免疫组化SP法检测CD147与MMP-2蛋白的表达,RT-PCR检测CD147mRNA表达,并探讨CD147表达与患者预后的关系。结果:观察组中CD147阳性表达率为75.71%,MMP-2阳性表达率74.29%,两者存在相关性(r=0.870,P<0.05)。高级别组和低级别组CD147mRNA表达水平均强于对照组,差异有统计学意义(均P<0.05),且高级别高于低级别组,差异有统计学意义(P<0.05)。复发患者CD147阳性表达率显著高于非复发患者,差异有统计学意义(P<0.05);生存时间<5年患者CD147阳性表达率显著高于生存时间≥5年患者,差异有统计学意义(P<0.05)。结论:人脑胶质瘤中CD147和MMP-2的表达与肿瘤恶性程度和患者预后密切相关,可为患者临床预后判断和临床治疗提供参考。Objective: To explore the expression and clinical significance of CD147 and MMP-2 in human gliomas,and to provide evidence for clinical treatment. Methods: The specimens of glioma of 70 patients, who underwent surgical resection and were confirmed by pathological diagnosis in First Affiliated Hospital of Jiamusi University from January 2010 to May 2015, were chosen as observation group, divided into low-level group (n=31) and high-level group (n=39) according to the WHO classification. Another 20 patients of specimens of brain trauma, given inner pressure resection, as control group. The expressions of CD147 and MMP-2 protein were detected by immunohistochemical SP method, and the expression of CD147mRNA, by RT-PCR. Results: In the observation group,the positive expression rate of CD147 and MMP-2 were 75.71% and 74.29% respectively,there was a correlation between CD147 and MMP-2 (r-=0. 870,P〈0.05). The expressions of CD147mRN in the high-level group and low-level group were higher than that in the control group, the difference was statistically significant (P 〈0.05); furthermore, the high-level group was higher than low-level group, the difference was statistically significant (P〈0.05). The positive expression rate of CD147 in the patients with recurrence was significantly higher than that in the patients without recurrence, the difference was statistically significant (P〈0.05). The positive expression rate of CD147 in the patients of survival time 〈5 years was significantly higher than that in the patients of survival time ≥ 5 years, the difference was statistically significant (P〈0.05). Conclusions: The expressions of CD147 and MMP-2 are closely related with malignant degree and prognosis of the patients with human gliomas, which can provide a reference for clinical prognosis and clinical treatment of the patients with human gliomas.
