摘 要:目的建立种植体支抗承载Ⅱ类颌间牵引进行上下颌骨矫形治疗的动物模型。方法选取替牙期雄性恒河猴为实验动物,在自行研制的导板引导下,在上下颌骨的牙槽间隔区植入微螺钉,并以微螺钉作为支抗,通过连接在上下微螺钉之间的钛镍拉簧施加对上颌骨向后、下颌骨向前的牵引力,持续作用6个月;通过动物口内观察、模型分析及X线头影重叠描记的方法观察施力结果。结果微螺钉安全植入并在实验期间保持稳定;实验动物的双侧磨牙关系由Ⅰ类变为Ⅲ类,切牙由深覆盖变为反殆;上颌骨前界有轻度向下向后移位,下颌骨及下牙弓有明显向下向前变化。结论本实验建立了种植体支抗承载Ⅱ类颌间牵引上下颌骨的动物模型,结果表明种植体支抗承载Ⅱ类颌间牵引可为矫治Ⅱ类颌骨不调的治疗提供一种方法。Objective To establish an animal model for maxillary orthopedic treatment with miniserew-anehored class Ⅱ elastics. Methods Male rhesus monkeys with mixed dentition were selected as the experimental animals. Four miniscrews were implanted in the alveolar bone of maxilla and mandible of each animal as the anchorage for traction of maxilla and mandible, guided by self- developed templates. NiTi closure coils were attached to the miniscrews. By opening the closure coil, a backward force was exerted on each side of the maxilla and forward force on the mandible for 6 months. Intraoral models and cephalometrics were done to assess the effects. Results The miniscrews were implanted safely and kept stable during the experiment. The molar relationship of both sides changed from Class I to Class Ill, and the overjet of anterior dental relationship changed from overjet to crossbite. The anterior border of maxilla was backward and downward and the mandible and lower dentition kept growing forward and downward. Conclusions The animal model for traction of maxilla and mandible with miniscrews anchorage has been established. The results indicate that miniscrew-anchored class Ⅱ elastics can apply as a method in maxillary orthopedic treatment.
