胸腔镜辅助小切口施行肺血管-支气管成形术治疗肺癌的可行性及效果

摘　　要：目的探究胸腔镜辅助小切口施行肺血管-支气管成形术治疗肺癌的可行性及效果。方法回顾性分析我院于2011年12月-2012年12月期间收治的在胸腔镜辅助小切口下施行肺血管-支气管成形术的肺癌患者共60例,设为观察组,同期选取采用常规后外侧切口施行肺血管-支气管成形术的肺癌患者60例,设为对照组,比较两组患者的治疗疗效。结果观察组患者手术时间、住院时间和手术后胸管的留置时间均显著低于对照组,差异具有统计学意义(P<0.05);观察组患者术后1年和3年的生存率与对照组患者无显著差异,差异不具有统计学意义(P>0.05);观察组患者术后并发症的发生率显著低于对照组,差异具有统计学意义(P<0.05)。结论胸腔镜辅助小切口施行肺血管-支气管成形术治疗肺癌效果显著,与常规后外侧切口手术疗效相当,而且手术时间、住院时间和导管留置时间比常规手术短,对患者损伤小,术后并发症发生率低,安全性好,临床上具有可行性,值得进一步推广应用。Objective To explore the feasibility and effectiveness of pulmonary vascular bronchial angioplas-ty in treatment of lung cancer with small incision assisted by video assisted thoracic surgery. Methods A retrospec-tive analysis was conducted in 60 lung cancer patients received thoracoscope assisted small incision performed pulmo-nary vascular bronchial angioplasty from December 2011 to December 2012 as the observation group. Another 60 lung cancer patients received conventional lateral incision performed pulmonary vascular and bronchial angioplasty were taken as the control group. Their curative effect was compared. Results In the observation group, operation time, hospital stay and operative chest tube indwelling time were significantly lower than those in the control group （ P〈0. 05）. The 1 and 3 years survival rate showed no significant difference between the two groups （P〉0. 05）. The in-cidence of postoperative complications was significantly lower in the observation group than in the control group （ P〈0. 05 ） . Conclusion VATS assisted small incision underwent pulmonary vascular bronchial angioplasty has good cur-ative effect in treatment of patients with lung cancer, which is similar with conventional lateral incision surgery, but it has the advantages of shorter operation time, hospital stay and catheter indwelling time, lower postoperative complica-tion incidence and higher safety.
