摘 要:目的探讨血清骨代谢标志物组合模型对诊断肺癌骨转移的价值。方法选择186例临床患者,分为肺癌伴骨转移组、肺癌不伴骨转移组和良性疾病对照组,分析各组间1型胶原羧基端肽β-特殊序列(BCROSS)、1型前胶原氨基端前肽(TPINP)、骨钙素N端中分子片段(N-MID)、甲状旁腺素(PTH)、维生素D等骨代谢标志物及碱性磷酸酶(ALP)、钙(Ca)、磷(P)、角蛋白19等指标的差异,通过Logistis回归分析,建立组合模型,采用ROC曲线分析其对肺癌骨转移的诊断性能。结果肺癌伴骨转移组BCROSS、TPINP、ALP、Ca水平明显高于肺癌不伴骨转移组,肺癌伴骨转移组NMID、BCROSS、TPINP、ALP、角蛋白19水平明显高于对照组,肺癌不伴骨转移组BCROSS、TPINP、ALP、角蛋白19水平明显高于对照组,差异均有统计学意义(P<0.05)。结论骨代谢标志物组合模型对诊断肺癌骨转移具有重要的临床意义。Objective To investigate the value of joint models of bone metabolism markers for diagnosing bone metastasis of lung cancer. Methods 186 clinical patients were collected and divided into lung cancer with bone metastases group,lung cancer without bone metastases group,and a control group of benign diseases. To compare the differences of β- Cross Laps( BCROSS),total procollagen type 1 N- terminal peptide( TPINP),N- terminal- mid fragment of osteocalcin( N- MID),parathyroid hormone( PTH),vitamin D,alkaline phosphatase( ALP),calcium( Ca),phosphorus( P) and cytokeratin 19 in each group. Through Logistis regression analysis,the combination model was established,and the ROC curve was used to analyze the diagnostic performance of lung cancer with bone metastasis. Results The levels of BCROSS,TPINP,ALP and Ca in bone metastasis group were significantly higher than those in non- bone metastasis. The levels of NMID,BCROSS,TPINP,ALP and cytokeratin 19 in bone metastasis group were significantly higher than that of the control group. The levels of BCROSS,TPINP,ALP and cytokeratin 19 in non- bone metastasis group were significantly higher than those in the control group. These differences were of statistical significance( P 0. 05). Conclusion The joint models of bone metabolism markers have important clinical significance for diagnosing of lung cancer with bone metastasis.
