摘  要:目的建立激光诱导光化学反应制备大鼠正中神经定量损伤模型并验证该模型的可行性。方法根据右侧正中神经激光照射时间将16只SD大鼠随机分为4组:对照组,10s组,20s组,40s组。在大鼠尾静脉注射赤藓红溶液(32.5mg/kg体质量)后5min在上臂近端显露正中神经长约5mm予以激光照射损伤。激光照射参数:532nm波长,170mW强度,200μm直径。损伤后1、2、3、4、5、6、7d,及2、3、4、5、6周对大鼠抓握能力、舔足反应进行行为观察;在损伤后第二周每组切取2只大鼠损伤段以远正中神经纤维进行甲苯胺蓝染色,观察有无脱髓鞘变化。结果所有实验大鼠术后均存活,完成各种行为观察。损伤后1~7d各实验组大鼠损伤一侧均出现抓握能力受限、舔足频繁等表现,对照组未出现相应损伤表现。各个实验组中,40s组大鼠行为表现较突出。2~4周时间段,各组观察功能状态持续下降。损伤后5~6周观察10、20s组大鼠抓握能力有所恢复,轻度受限;而40s组一只大鼠有抓握动作的部分恢复,另一只大鼠则无明显恢复。组织学观察发现在2周时有髓神经纤维均出现脱髓鞘变化,40s组脱髓鞘尤为明显。而对照组无相应变化。结论光敏剂注射后,不同剂量的激光照射所致的神经损伤可累及运动、感觉和交感功能,而且损伤程度不同。激光诱导光化学反应制备大鼠单根神经定量损伤模型成功率较高,稳定性好,具有损伤定量的优势。Objective To establish laser induced photochemical quantitative median nerve injury model in rats and validate the feasibility of this model. Methods Sixteen Sprague-Dawley rats were randomly divided into 4 groups based on the duration of laser irradiation to their median nerves: control group,10s group, 20 s group and 40 s group. A 5 mm segment of the right median nerves at the proximal upper arm was exposed 5 minutes after Etythrosine injection (32.5 mg/kg) via the tail vein. The exposed nerve was uniformly irradiated by a laser with 532 am wavelength, 170 mW intensity and 200 tun beam diameter. Behavior of the rats, including grasping ability and foot licking, were observed daily in the first week and weekly for 5 more weeks after the injury. Two weeks after the injury, median nerve samples distal to the laser irradiation site were harvested from 2 rats of each group for tolnidine blue staining to observe the histological changes. Results All the rats survived the procedure and various behaviors were observed. Limitation of grasping ability and frequent licking of the foot on the injury side were observed in rats of all three experimental groups from day 1 to day 7. This observation was most remarkable in rats of the 40 s group. These behaviors were not seen in tats of the control group. From 2 to 4 weeks, the functional status continued to decline. From 5 to 6 weeks, rats in the 10 s and 20 s groups started to show recovery of grasping activity, whereas in the 40 s group one rat had partial grasping activity while the other one had no obvious activity. Histological examination of nerve samples 2 weeks after laser irradiation showed demyelination changes of the myelinated fibers, being most pronounced in the 40 s group while no changes in the control group. Conclusion After photo initiator injection, laser irradiation with different doses can cause varying degrees of nerve injury that involves motor, sensory and sympathetic functions of the nerve.
