摘 要:[目的]探讨血浆胱抑素C(CysC)对脑梗死患者静脉溶栓疗效的影响。[方法]选择2010年6月至2015年6月于本院住院并接受静脉溶栓治疗的脑梗死患者80例作为研究对象。以血浆CysC含量〉1.1mg/L的32例患者为高含量纽,≤1.1mg/L的48例患者为低含量组。比较两组患者的疗效、美国国立卫生院神经功能缺损量表(NIHSS)评分和不良预后情况。[结果]低含量组患者有效率为85.42%显著高于高含量组的75.00%,差异有统计学意义(P〈0.05);溶栓前,两组患者的NIHSS评分比较差异无统计学意义(P〉0.05),溶栓1d、14d、28d后低含量组患者的NIHSS评分低于高含量组,差异有统计学意义(P〈0.05);低含量组患者再发脑梗死、症状性脑出血、非症状性脑出血、死亡等不良预后的发生率明显低于高剂量组,差异有统计学意义(P〈0.05)。[结论]血浆CysC可用来预测脑梗死患者静脉溶栓的疗效,其含量越低,脑梗死患者静脉溶栓后的疗效越好,安全性越高。[Objective]To study the effect of Cystatin C (Cys C) on the treatment of intravenous thrombolysis in patients with cerebral infarction. [Methods] A total of 80 cases of patients with cerebral infarction who received intravenous thrombolysis at our hospital from June 2010 to June 2015 were recruited. According to the content levels of serum Cys C, the patients were divided into two groups: Serum Cys C 〉 1.1 mg/L were categorized as the high content group of patients ( n = 32) and patients with Cys C ≤ 1.1 mg/L were categorized as the low content of group ( n = 48). The curative effects of patients in the two groups were scored based on the National Institutes of Health Stroke Scale (NIHSS). And occurrences of bad prognosis were compared between patients of the two groups. [Results]The efficacy rate of treatment in the low content group was 85.42%, significantly higher than that of the high content group (75.00 %); the difference was statistically significant ( P 〈0.05). Before treatment,the NIHSS scores of patients in the two groups showed no statisticatly significant difference ( P 〉0.05). After 1 d, 14 d, and 28 d of fibrinolytic, the NIHSS scores of patients in the low content group were significantly lower than those of the high content group;the difference was statistically significant ( P 〈0.05). The reoccurrence of cerebral infarction, symptomatic cerebral hemorrhage, asymptomatic cerebral hemorrhage, and incidence of adverse outcomes such as death were significantly lower in the low content group than in the high content group; the difference had statistical significance ( P 〈0.05). [Conclusion]Cys C can be used to predict the efficacy of intravenous thrombolysis in patients with cerebral infarction. The lower the level of C Cys in patients with cerebral infarction, the better the efficacy of the treatment of cerebral infarction and the higher the safety.
