不同菌型幽门螺杆菌感染消化性溃疡血清中T细胞亚群的变化及临床意义

摘　　要：目的检测不同菌型幽门螺杆菌(Hp)感染消化性溃疡(PU)患者血清中T细胞亚群的变化以及白细胞介素-17(IL-17)、转化生长因子-β(TGF-β)的表达。方法按照随机数字表法选择经胃镜检查确诊的PU患者共88例,同期纳入30例健康体检者作为对照组;检测Hp感染状况并分型,检测所有研究对象外周血T淋巴细胞亚群活性、IL-17、TGF-β含量。结果Hp阳性组患者CD4+和IL-17水平明显高于Hp阴性组,而TGF-β水平明显低于Hp阴性组(P<0.01);Ⅱ型Hp组患者CD4+和IL-17水平明显低于Ⅰ型Hp组,TGF-β水平明显高于Ⅰ型Hp组(P<0.01);在Ⅰ型Hp感染组,十二指肠球部溃疡(DU)患者的CD4+和IL-17表达水平明显高于胃溃疡(GU)患者,而TGF-β水平明显低于GU患者(P<0.01);在Ⅱ型Hp组,CD4+、IL-17和TGF-β水平在DU与GU间的表达水平差异无统计学意义(P>0.05)。结论血清CD4+T细胞增高及其细胞因子IL-17和TGF-β的表达异常可能在Hp感染PU中发挥了重要调节作用。Objective To investigate changes of T cell subset in serum of peptic ulcer（PU）patients with infection of different types of bacteria helicobacter pylori（Hp）and expressions of interleukin（IL）-17 and transforming growth factors（TGF）-β.Methods Eighty-eight patients with PU confirmed by gastroscope examination were chosen ifor the study,and at the same time 30 healthy examined peo-ple as control group.Types of Hp infection were detected.Peripheral blood T lymphocyte subsets and NK cells activities were detected by flow cytometer in all cases.Levels of IL-17 and TGF-βwere detected by Elisa method.Results CD4 ＋ and IL-17 in Hp positive group were obviously higher,while TGF-βwas evidently lower than Hp negative group（P 〈0.01）.CD4 ＋and IL-17 in Hp typeⅡgroup were obviously lower,while TGF-βwas evidently higher than HpⅠgroup （P 〈0.01）.In Hp typeⅠgroup,CD4 ＋ and IL-17 of duodenal ulcer（DU）patients were obviously higher,while TGF-βwas evidently lower than gastric ulcer（GU）patients（P 〈0.05）.In Hp typeⅡgroup,CD4 ＋,IL-17 and TGF-βin DU and GU were not significantly different（P 〉0.05）.Conclusions Increase in serum CD4 ＋ T cell and abnormal expressions of IL-17 and TGF-βmay play an important regulating role in PU patients with Hp infection.
