不同分型短暂性脑缺血发作患者的血压变异性研究

摘　　要：目的探讨不同分型TIA患者的血压变异性。方法采集246例TIA患者和246例同期健康者的临床资料。根据受累血管的不同,将TIA患者分为前循环系统TIA组（AC-TIA）160例和后循环系统TIA组（PC-TIA）86例。24 h动态血压仪监测血压值和血压变异性。结果与对照组比较,PC-TIA组和AC-TIA组收缩压和舒张压明显升高（P〈0.05~0.01）;与AC-TIA组比较,PC-TIA组收缩压和舒张压明显升高（均P〈0.05）。与对照组比较,PC-TIA组和AC-TIA组24 h、白天、夜间平均收缩压和舒张压以及脉压指数均明显升高（P〈0.05~0.01）;与AC-TIA组比较,PC-TIA组24 h、白天、夜间平均收缩压和舒张压以及脉压指数均明显升高（均P〈0.05）。与对照组比较,PC-TIA组和AC-TIA组24 h、白天、夜间收缩压变异性明显增大（P〈0.05~0.01）;与AC-TIA组比较,PC-TIA组24 h、白天、夜间收缩压和舒张压变异性均明显增大（均P〈0.05）。结论短暂性脑缺血发作患者的血压变异性尤其是收缩压变异性明显增大,不同分型短暂性脑缺血发作患者的血压变异性存在差异。Objective To investigate the blood pressure variability（ BPV） between different subtypes of TIA.Methods The clinical data of 246 patients with TIA and another 246 physically healthy subjects were collected.Based on different involvement of blood vessels in TIA,these patients were divided into two groups： anterior circulation TIA（ AC-TIA） group（ n = 160） and posterior circulation TIA（ PC-TIA） group（ n = 86）. Their blood pressure value and variability were detected by 24 h ambulatory monitor. Results Compared with control group,SBP and DBP were significantly increased in PC-TIA and AC-TIA group（ P〈0. 05- 0. 01）. Compared with AC-TIA group,those were significantly increased in PC-TIA group（ all P〈0. 05）. Compared with control group,24 h mean SBP and DBP,daytime mean SBP and DBP,night mean SBP and DBP,pulse pressure index were significantly increased in PC-TIA and AC-TIA group（ P〈0. 05- 0. 01）. Compared with AC-TIA group,those were significantly increased in PC-TIA group（ all P〈0. 05）. Compared with control group,BPV of 24 h mean SBP,BPV of daytime mean SBP,BPV of night mean SBP were significantly increased in PC-TIA and AC-TIA group（ P〈0. 05- 0. 01）.Compared with AC-TIA group,BPV of 24 h mean SBP and DBP,BPV of daytime mean SBP and DBP,BPV of night mean SBP and DBP were significantly increased in PC-TIA group（ all P〈0. 05）. Conclusions The variability of TIA patients＇ blood pressure,especially of SBP,is obviously higher than that of the healthy population. There are statistical differences in BPV of different TIA groups.
