血清γ-谷氨酰转肽酶水平与2型糖尿病患者下肢动脉粥样硬化的关系

摘　　要：目的探讨正常范围内血清γ-谷氨酰转肽酶（GGT）水平与2型糖尿病患者下肢动脉病变（LEAD）的关系。方法选取2型糖尿病患者406例，根据GGT四分位数将患者分为4组，分别为Q1组（3IU／L≤GGT≤18IU／L）100例，Q2组（18IU／L〈GGT≤29IU／L）102例，Q3组（29IU／L〈GGT≤40IU／L）101例，Q4组（40IU／L〈GGT≤50IU／L）103例。采用Logistic回归分析研究血清GGT与LEAD的关系。结果Q1、Q2、Q3、Q4组LEAD的发病率分别为21．00％、28．43％、39．60％和44．66％，与Q1组相比，Q3、Q4组LEAD患病率明显升高（P均〈0．05），在调整一般资料和生化指标后，Q4组LEAD发生率仍升高[比值比（OR）=2．38，95％可信区间（CI）1．01-5．61，P〈0．05]。结论随着血清GGT水平升高，2型糖尿病患者LEAD的发生率增加。Objective To explore the relationship between the serum levels of γ-glutamyhrans- ferase（GGT） in normal range and lower extremity arterial disease （LEAD） in type 2 diabetes mellitus （T2DM） patients. Methods 406 inpatients with T2DM were recruited in this study. Based on the quartile of serum GGT level, all patients were divided into four groups ： 100 cases in group Q1 （ 3 IU/L ≤ GGT ≤18 IU/L） ,102 cases in group Q2（ 18 IU/L 〈 GGT≤29 IU/L） ,101 cases in group Q3（29 IU/L 〈 GGT≤ 40 IU/L） and 103 cases in group Q4（40 IU/L 〈 GGT≤50 IU/L）. The Logistic regression analysis was used to analyze the relationship between GGT and LEAD. Results The prevalence of LEAD was 21.00% in Q1,28.43% in Q2,39.60% in Q3 and 44.66% in Q4 respectively. Compared with Q1,the LEAD incidence was significantly higher in Q3 and Q4 （ P 〈 0.05 ）. After adjusting clinical characters and biochemical data,the LEAD prevalence was still higher in Q4（ OR 2.38,95% CI 1.01-5.61 ,P 〈0.05）. Conclusion The prevalence of LEAD in T2DM patients increases accompanied with the rise of GGT. Elevated serum GGT in normal range is associated with LEAD in T2DM.
