摘 要:目的:探讨黑质"燕尾征"对帕金森病(parkinson's disease,PD)的诊断价值。方法:连续纳入已被英国脑库帕金森病临床诊断标准确诊的PD患者50例,30例年龄相仿的健康对照来自来医院体检且无神经系统疾病的自愿者。使用3.0T的MR磁敏感成像序列(SWI)扫描中脑黑质。以"燕尾征"缺失作为PD影像学诊断判断标准,评估诊断PD的敏感度、特异度、阳性预测值、阴性预测值和一致性。结果:3.0T的MR磁敏感成像序列诊断PD的敏感度达100%,特异度96.3%,阳性预测值97.9%,阴性预测值100%,一致性98.6%。结论:3.0T-MRI磁敏感成像上的燕尾征可以作为一种评定黑质变性的指标。Objective To investigate the diagnostic value of "Swallow Tail" appearance demonstrated in substantia nigra in pa- tients with Parkinson disease. Methods Fifty patients fulfilling the UK Brain Bank criteria for PD were recruited from Xian- gyang Hospital affiliated to Hubei University of Medicine.Thirty age-matched controls were recruited from voluntary healthy people without nervous system diseases.All the subjects in mesencephali substantia nigra were scanned by using 3.0T sus- ceptibility-weighted (SWI) MRI.The diagnostic accuracy was assessed by taking absent of "Swallow Tail" appearance as judgment standard of PD. Results The sensitivity, specificity, positive predictive value ( PPV ), negative predictive value (NPV) and consistency of diagnostic accuracy with SWI MRI was 100%,96.3%,97.9%, 100% and 98.6%,respectively. Conclusion The "Swallow Tail" appearance of substantia nigra could be an index for diganosising Parkinson disease with 3. 0T SWI-MRI.
