角膜生物力学属性对角膜塑形镜效果的影响

摘　　要：目的探讨角膜生物力学属性对角膜塑形镜早期治疗效果的影响。方法选取2013年1月至4月于哈尔滨医科大学附属一院眼科医院视光学中心验配角膜塑形镜的患者41例(41眼)。在戴镜前、戴镜1天、1周时测量屈光力、角膜曲率、中央角膜厚度、角膜滞后量以及角膜阻力因子。采用配对t检验和相关性分析的方法对数据进行分析。结果配戴角膜塑形镜后等效球镜和角膜曲率均明显降低,戴镜1夜后变化即有统计学意义,P<0.05。戴镜1周内角膜生物力学参数CH、CRF值呈下降趋势,P<0.05。戴镜前CH值与戴镜1夜后SE下降程度呈负相关性,P<0.05,与其他参数变化无相关性,P>0.05。CRF值与戴镜后SE、Kf和Ks的变化均无相关性,P>0.05。结论角膜生物力学参数值低的患者,配戴角膜塑形镜早期近视矫正的速度快。Objective To investigate the effects of corneal biomechanical properties on treatments of Orthokeratology lens in early stages. Methods Choose 41 cases （41 eyes） which obtain Orthokeratology lenses at optometry center of ophthalmology department at the first affiliated hospital of Harbin medical university from January, 2013 to April, 2013. Collect the data which include refractive power, corneal curvature, corneal central thickness, corneal hysteresis and corneal resistance factors. Apply paired sample test and correlation to analyze. Results Spherical equivalent and corneal curvature significantly reduced after wearing Orthokeratology and changes were statistically significant after wearing overnight （P 〈 0.05）. Within one week wearing corneal biomechanical parameters CH and CRF value showed a downward trend, and the difference was statistically significant （P 〈 0.05）. After wearing overnight the degree of decline of SE and CH value was negatively correlated and with no correlation between changes in other parameters （P 〉 0.05）. Three are no correlation between CRF value and changes in SE, Kf and Ks after wearing Orthokeratology （P 〉 0.05）. Conclusion For the patients who have low values of corneal biomechanical parameters, the effect of wearing Orthokeratology lenses in early stages is more rapid.
