视觉诱发电位（VEP）检查在经颅视神经管减压手术前后的临床应用

摘　　要：目的：探讨视觉诱发电位（VEP）检查在经颅视神经管减压手术的视神经损伤患者的临床观察。方法对41例经颅视神经管减压手术的视神经损伤患者进行术前、术后三个月V E P检查。结果术前，视神经损伤眼VEP与健侧眼比较，P100波幅明显低，无光感组较另外三组下降明显，无光感组潜伏期延长明显；术后，无光感组外，另外三组P100潜伏期延长明显、波幅增高，术前视力、术后视力与术前P100波幅下降程度有负相关性，术后视力与术后P100波幅下降程度有负相关性，VEP熄灭型手术效果差。结论 VEP是一种客观、定量评定视神经损伤的方法，能早期诊断视神经损伤，对手术疗效及预后判断提供重要的客观依据。Objective To study patients with optic canal decompression by visual evoked potential （VEP） .Methods 41 eyes in the 41 patients of optic canal decompression were examined with VEP before and after transcranial approaches optic canal decompression .All of them were followed up 3 months later and VEP test were examined .Results Compared with another eye of normal controls eyes ,the P100 amplitude of VEP were obviously lower ,the most obvious decline were in patients without light .The P100 latency of VEP in patients without light were significantly delayed ,while other patients were slight delayed .There were significant delayed in P100 latency and obvious increased in P100 amplitude of VEP after operation except patients without light .The visual acuity was rrelated with amplitude of P100 wave in preoperations and postoperations .Conclusion The VEP was an objective ,quantitative method to evaluate the optic nerve .The VEP could diagnosis the optic nerve injuries and was an objective examining method on assessing the efficacy and prognosis .
