摘  要:目的检测氧自由基和细胞间黏附分子-1(ICAM-1)在蛛网膜下腔出血(SAH)动物模型上的表达,探讨其与脑血管痉挛(CVS)时相变化的关系。方法采用枕大池二次注血法制做大鼠SAH模型,各组动物在处死前测脑血流量(rCBF),处死后切取基底动脉,一部分行HE染色、免疫组化染色及Western blot法检测ICAM-1的表达;其余制成匀浆,黄嘌呤氧化酶法测定超氧化物歧化酶(SOD)活性,硫代巴比妥酸(TBA)法测定丙二醛(MDA)含量。结果 SAH组rCBF呈双相变化,SAH组1d后基底动脉出现管腔狭窄,血管壁ICAM-1表达增强,3d最为明显,且SOD活性与对照组比在1h达到最低(P<0.01),至5d仍与对照组有差异(P<0.05);SAH组的MDA含量与SOD呈反向变化。结论大鼠SAH后出现双相性CVS,氧自由基参与急性期CVS的发展,ICAM-1参与迟发期CVS的发展。Objective To evaluate the expression and roles of oxygen free radical and intercellular adhesion moleeule-1(ICAM-1) in different phases of cerebral vasospasm (CVS) following subarachnoid hemorrhage(SAH) in rat models. Methods The rat model of SAH was completed by double injection autogeneic arterial blood into cisterna magna. Regional cerebral blood flow (rCBF) was measured for each rat before sacrificing. The basilar arteries were isolated, fixed, embedded, followed by Hematine-Eosin (H-E) stain and immunohistochemical detection, and the expression of ICAM-1 protein in blood vessels was detected by Western blot. The remaining parts of the Willis circle were isolated and homogenized for detection of activity of hepatocuprein(SOD) using xanthine oxidase method and content of malondialdehyde(MDA) using thiobarbituric acid method, respectively. Results The measured rCBF level in SAH group exhibited considerable and dual-phase variation. H-E stain revealed twice marked narrowing in the lumens of basilar arteries in 1 hour group and 3 days following SAH. The significant pathological changes were found in the vessel wall of basilar arteries after 1 day, accompanied with gradually increasing ICAM-1 immunoreactivity and protein levels, which waked at 3 d. The activity of SOD in SAH group decreased to the lowest at 1 h compared with control group(P〈0.01), from then increased but remained lower than in control group (P〈0.05) until 5 d after SAH. On the contrary, the content of MDA in SAH group increased to the highest level at 1 h compared with control group (P〈0.01), from then decreased but remained higher than in control group (P〈0.05) untill 5 d after SAH. Conclusion Acute and delayed CVS occurred after SAH, oxygen free radical mainly participates the development of acute CVS, while the ICAM-1 might play pivotal role in the phase of delayed CVS.
