探讨Cyfra21-1和CEA及CA125联合检测对食管癌诊断及分期的意义

摘　　要：目的:探讨外周血肿瘤相关抗原Cyfra21-1、CEA及CA125联合检测对食管癌的诊断价值。方法:电化学发光免疫分析法测定215例食管癌患者与100名健康体检者外周血清Cyfra21-1、CEA和CA125的水平,t和χ2检验进行数据统计分析。结果:食管癌组血清Cyfra21-1、CEA和CA125的水平及阳性率分别为(5.8±4.6)ng/m L与45.6%、(7.8±3.4)ng/m L与33.5%及(12.6±4.3)U/m L与34.0%,均显著高于健康对照组的(4.0±3.5)ng/m L与13.0%、(4.9±1.3)ng/m L与9.0%、(9.3±3.4)U/m L与7.0%,(P<0.05)。随着临床分期的递增,血清Cyfra21-1、CEA和CA125的水平和阳性率也不同程度上升。食管癌患者血清Cyfra21-1、CEA和CA125联合检测可以提高肿瘤检测灵敏度(81.4%),与单项检测比较差异有统计意义(P<0.05)。结论:联合检测食管癌血清中Cyfra21-1、CEA和CA125水平变化,有助于提高食管癌诊断的灵敏度。Objective To investigate the serum levels of Cyfra21-1, CEA and CA125 in peripheral blood and explore their clinical significance on diagnosis of patients with esophageal cancer. Methods The serum levels of Cyfra21 - 1, CEA and CAi25 in peripheral blood of 215 patients with esophageal cancer and 100 healthy volunteers were detected by electrochem- iluminescence,then the results was analyzed by t test and x2 test.Results The serum levels of Cyfra21-1 ,CEA and CA125 in esophageal cancer patients were （5.8±4.6）ng/mL, （7.8±3.4）ng/mL and （12.6±4.3） U/mL,and in healthy volunteers was （ 4.0±3.5 ） ng/mL, （ 4.9±1.3 ） ng/mL, （ 9.3 ±3.4 ） U/mL, respectively.The postive rates of Cyfra21 - 1, CEA and CA 125 in esophageal cancer patients were 45.6% ,33.5% and 34.0%, in healthy volunteers were 13.0% ,9.0% and 7.0% ,respectively.The serum levels and postive rates of Cyfra21-1, CEA and CA125 in esophageal cancer patients were significantly higher than those of healthy volunteers（P〈0.05 ）. Following the increasing of clinical stage, the serum levels and positive rate of Cyfra21 - 1, CEA and CA125 were increased with different extent.The sensitivity of combined detection of Cyfra21-1, CEA and CA125 was high to 81.4% in patient with esophageal cancer,which was obviously superior to the single detection （P〈0.05） .Conclusion Combined detection of serum Cyfru21-1, CEA and CA125 levels could improve the sensitivity of e- sophageal cancer diagnosis.
