恶性淋巴瘤诊断中Ig/TCR基因重排的分子病理检测分析

摘　　要：目的探讨B细胞性淋巴瘤中Ig基因重排和T细胞性淋巴瘤中TCR基因重排的特点。方法回顾性分析213例恶性淋巴瘤和24例淋巴反应性增生组织样本,利用3730毛细血管电泳对提取的DNA进行PCR扩增,检测B细胞性淋巴瘤中Ig基因重排和T细胞性淋巴瘤中TCR基因重排。结果 B细胞性淋巴瘤中IgHA的总阳性率为43.21%,IgHB的总阳性率为29.63%,IgHC的总阳性率为30.25%,IgHD的总阳性率为16.67%,IgHE的总阳性率为3.09%,IgLκA的总阳性率为42.59%,IgLκB的总阳性率为23.46%,IgLλA的总阳性率为25.93%,IgH(A+B+C+D+E)+IgLκ(A+B)+IgLλA的总阳性率为88.27%;T细胞性淋巴瘤中TCRB-A的总阳性率为31.37%,TCRB-B的总阳性率为58.82%,TCRB-C的总阳性率为23.53%,TCRG-A的总阳性率为60.78%,TCRG-B的总阳性率为21.57%,TCRD的总阳性率为29.41%,TCRβ+TCRγ+TCRδ的总阳性率为78.43%。结论Ig基因重排和TCR基因重排分别对B细胞性和T细胞性淋巴瘤的诊断具有重要价值。Objective To analyze the character of the Ig gene rearrangement in B cell lymphoma and TCR gene rearrangement in T cell lymphoma. Methods A retrospective analysis of 213 cases of samples with malignant lymphoma and 24 cases of samples with reactive lymphoid hyperplasia, which were used to extract DNA for PCR amplification by 3730 capillary electrophoresis and had detected Ig gene rearrangement in B cell lymphoma and TCR gene rearrangement in T cell lymphoma. Results Among B cell lymphoma, the total IgHA positive rate was 43.21 %, the total IgHB positive rate was 29.63%, the total IgHC positive rate was 30.25%, the total IgHD positive rate was 16.67%, the total IgHE positive rate was 3.09% ,the total IgLκA positive rate was 42.59%, the total IgLκB positive rate was 23.46%, the total IgLκB positive rate was 25.93% and , the total IgH （A＋B＋C＋D＋E） ＋IgLκ（A＋B）＋IgLλA positive rate was 88.27%. Among T cell lymphoma, the total TCRB-A positive rate was 31.37%, the total TCRB-B positive rate was 58. 82%, the total TCRB-C positive rate was 23.53%, the total TCRG-A posi- tive rate was 60.78%, the total TCRG-B positive rate was 21.57%, the total TCRD positive rate was 29.41% and the total TCRβ＋TCRγ-TCRδ positive rate was 78.43 %. Conclusion It has an important value for Ig gene rearrangement in B cell lymphoma and TCR gene rearrangement.
