不随意运动型脑瘫早期黄疸28例

摘　　要：目的 探讨不随意运动型脑瘫与新生儿期高胆红素血症的关系。方法 对有明显新生儿黄疸史的不随意运动型脑瘫患儿28例,通过问卷调查,记录新生儿期高胆红素血症的原因、程度、临床特点及治疗情况。结果 28例患儿高胆红素血症的主要原因为：新生儿溶血病8例,占28.64%;母乳性黄疸2例,占7.1%;早期感染4例,占14.3%;巨大头颅血肿3例,占10.7%;病因不明11例,占39.3%。早产儿4例。血清总胆红素〉525μmol/L者4例,438～525μmol/L者10例;350～438μmol/L者5例;263～350μmol/L者2例,胆红素不详者7例。黄疸明显加重时间为出生后（150±12）d,持续时间为出生后（923±5）h,21例患儿诊治过,但首次治疗时间均在出生后150 h左右。结论 严重的高胆红素血症是不随意运动型脑瘫主要危险因素,早期迅速低降低血胆红素浓度是减少不随意运动型脑瘫的关键。Objective To explore the involuntary sports cerebral palsy and neonatal period the relationship between high blood bilirubin. Methods There are obvious history of neonatal jaundice involuntary sports cerebral palsy 28 cases, through the questionnaire and records the neonatal period, the causes of high bilirubin concentration degree, clinical characteristics and treatment. Results 28 cases of children with high blood bilirubin： was the main cause of neonatal hemolysis disease 8 cases, accounting for 28.64%. Breast milk jaundice in 2 cases, accounting for 7.1%. Early infection, 4 cases （14.3%）. Big head haematoma （3 cases）, accounting for 10.7%. Unknown etiology in 11 cases, accounting for 39.3%. 4 cases of premature babies. Serum total bilirubin 〉 30 mg/dL, 4 cases of 25 mg in 10 - 30 mg/L, 20 - 25 mg mg/L in 5 cases, 15 -20 mg mg/L in 2 cases, 7 cases of bilirubin is unknown. Jaundice significantly increased after birth （150 ＋ 12） d, duration of （923 ＋ 5） h after birth, 21 cases were treated, but the treatment time of around 150 h after birth for the first time. Conclusion The serious high blood bilirubin is a major risk factor, not casual sport cerebral palsy early low quickly reduce blood bilirubin concentration is the key to reduce involuntary motile cerebral palsy.
