再次脑出血早发癫痫危险因素探讨

摘　　要：〔目的〕探讨再次脑出血早发癫痫的危险因素。〔方法〕对河南大学第一附属医院2010年1月至2015年6月收治的30例再次脑出血早发癫痫患者的年龄、性别、NIHSS评分、高血压病史、吸烟史、出血部位、出血量、电解质情况、外科治疗、外伤史等进行回顾性分析,对影响早发性癫痫发作的相关临床因素进行单因素分析和Logistic回归分析。〔结果〕30例患者中,单因素分析结果显示,年龄、性别、NIHSS评分、高血压病史、吸烟史与再次脑出血早发癫痫密切相关(P<0.05)。出血部位、出血量、电解质情况、外科治疗、既往外伤史与其无明显相关性(P>0.05)。〔结论〕高龄、男性、NIHSS评分较高、既往有高血压病史、吸烟史是再次脑出血早发癫痫的主要危险因素。出血部位、出血量、电解质情况、外科治疗、既往外伤史为不肯定因素。〔Objective〕To discuss with the risk factors of epilepsy as the first symptoms of cerebral hemorrhage again.〔Methods〕A total of 30 patients with epilepsy as the starting symptoms of cerebral hemorrhage patients again in our hospital from January2010 to June 2015 were collected. The patients＇ age, sex, bleeding parts, blood loss, bieeding times, electrolyte change, the score of NIHSS were studied retrospectively. The single factor analysis and the Logistic regression analysis were used to analysis the related clinical factors of affecting the early-onset seizures.〔Results〕With the 30 cases of patients, the single factor analysis results showed that, age, gender, number of bleeding, bleeding cortex were the independent risk factors of early onset epilepsy（ P〈0.05）. Bleeding again, electrolyte disorder, hypertension were the main factors of the early onset epilepsy in cerebral hemorrhage（ P〈0.05）. Logistic regression analysis showed that the bleeding, bleeding quantity oven, smoking, family history of epilepsy, history of alcoholism, drugs that may cause epilepsy, surgical treatment, traumatic brain injury. 〔Conclusion 〕The involvement of cortex, history of high blood pressure, cerebral hemorrhage medical history, electrolyte disorder, high scores of the degree of clinical neural function defect are the dangerous factors of early-onset seizures after cerebral hemorrhage. Not sure factor： The patients who were bleeding volume with cerebral hemorrhage need to alert highly the potential of early-onset epilepsy.
