全凭静脉麻醉与静吸复合麻醉在胸腔镜食道癌手术中应用比较

摘　　要：目的比较全凭静脉麻醉与静吸复合麻醉在胸腔镜食道癌手术中应用。方法选择择期胸腔镜食道癌根治术40例,随机分为观察组和对照组,各20例。观察组患者采用全凭静脉麻醉;对照组患者采用静吸复合麻醉。记录麻醉诱导前(T_0)、单肺通气后30 min(T_1)、单肺通气后60 min(T_2)的血压、动脉血氧饱和度(SPO_2)、动脉血氧分压(PaO_2)及动脉血二氧化碳分压(PaCO_2);同时记录麻醉时间、恢复自主呼吸时间、拔管时间及苏醒期躁动和恶心呕吐的发生率。结果两组患者麻醉时间、恢复自主呼吸时间、拔管时间差异均无统计学意义(P>0.05);观察组患者单肺通气后SPO_2、PaO_2高于对照组,而PaCO_2低于对照组(P<0.01);观察组患者苏醒期躁动及恶心呕吐的发生率低于对照组(P<0.05)。结论在胸腔镜食道癌手术中全凭静脉麻醉效果优于静吸复合麻醉。Objective To compare the application of total intravenous anesthesia and intravenous-inhalation combined anesthesia in thoracoscopic esophageal cancer operation. Methods Forty cases of thoraeoscopic esophageal cancer （ASA grade I-II） radical resec- tion with selective time were chosen and randomly divided into total intravenous anesthesia （Group A） and intravenous-inhalation com- bined anesthesia （Group B）. BP,SPO2 ,PaO2 and PaCO2 were recorded before anesthesia induction （To ） and 30 rain （TI ） and 60 min （ T2 ） ＇after OLV. Anesthesia time, eye opening time, extubation time and the incidence of agitation and nausea and vomiting in recovery pe- riod were also recorded. Results The anesthesia time, eye opening time and extubation time had no significant difference between the two groups （P 〉 0. 05 ）. Compared to Group B, Group A has increased level of SPO2 and PaO2 but lowered PaCO2 （P 〈 0. 01 ）. Moreover, the incidence of agitation and nausea and vomiting during recovery time in Group A was lower than Group B （P 〈 0. 05 ）. Conclusion Total intravenous anesthesia was superior to intravenous-inhalation combined anesthesia for thoracoscopie esophageal cancer resection.
