输尿管内留置巴德双J管治疗晚期恶性肿瘤继发输尿管梗阻效果观察

摘　　要：目的探讨输尿管内留置巴德双J管治疗晚期恶性肿瘤继发输尿管梗阻的临床效果。方法回顾性分析我院收治的54例晚期恶性肿瘤继发输尿管梗阻患者,分别通过输尿管内置入巴德双J管和普通双J管用于解除梗阻。其中置入巴德双J管患者20例(观察组),普通双J管患者34例(对照组)。比较两组患者血肌酐与血尿素氮下降幅度、并发症发生情况及输尿管梗阻复发率。结果经治疗1周后,两组患者血肌酐与血尿素氮均较治疗前下降明显(P<0.05),且观察组下降幅度大于对照组(P<0.05)。其少尿、无尿症状较前均有所缓解,尿液引流通畅。随访半年,观察组尿路感染、出血发生率均低于对照组(P<0.05);两组慢性疼痛发生率及输尿管梗阻复发率比较,差异均无统计学意义(P>0.05)。结论输尿管内留置巴德双J管是治疗晚期恶性肿瘤继发输尿管梗阻较为理想的方法,值得临床推广应用。Objective To investigate the clinical effect of double J ureteral catheter in the treatment of patients with ureteral obstruction after advanced malignant tumor. Methods A retrospective analysis of Fifty-four cases of advanced malignant tumor with u- reteral obstruction, respectively, through were placed the ureter double J tube and ureteral stent into the common double J tube to relieve the obstruction. The ureteral stent into the Bard double J tube 20 patients （observation group） and ureteral stent into the common doub- le J tube of 34 patients with （ control group）. Compared blood creatinine and blood urea nitrogen of the two groups and the incidence of complications and the recurrence rate of ureteral obstruction. Results After 1 weekg treatment, serum creatinine and blood urea nitro- gen of the two groups were significantly lower than those before treatment （ P 〈 0. 05 ） , and those of the observation group were signifi- cantly lower than the control group（ P 〈 0. 05 ）. Symptoms of oliguria and anuria have been eased, urine drainage unobstructed. During follow-up for six months, the incidence of urinary tract infection and bleeding in the observation group was lower than that in the control group（ P 〈 0. 05 ）. There was no significant difference between the two groups in the incidence of chronic pain and the recurrence rate of ureteral obstruction （ P 〉 0. 05 ）. Conclusion Indwelling double J tube in ureter is an ideal method for the treatment of ureteral ob- struction secondary to advanced malignant tumors, which is worthy of clinical application.
