不同人群前牙美学观点比较研究

摘　　要：目的探讨不同年份不同人群前牙美学审美观点的差异,为国内前牙美学修复工作提供参考。方法以上颌前牙的宽度比例、上颌切牙的宽长比例和微笑上唇线高度等常见前牙美学等特征为调查指标,分别对口腔临床医生和普通人群进行网络调查或现场访问调查。采用统计学方法对两组人群的调查结果进行比较分析。结果 2015年广州市口腔医生和普通人群的前牙审美观差异调查结果与2008年调查结果差异无统计学意义。口腔医生与普通人群在上颌前牙宽度比例、微笑位上唇线高度和上颌前牙临床牙冠倾斜角度三个方面差异具有统计学意义(P<0.05),在上颌切牙宽长比例(P=0.192)、颊间隙大小(P=0.176)两方面差异没有统计学意义;两组人群都认为完美微笑应该具备一定宽度的颊负性间隙(颊间隙率=0.7)。广州市不同收入水平、学历、工龄、常驻地和职称人群之间的前牙美学观点差异没有统计学意义(P>0.05)。结论口腔医生与普通人群前牙美学审美在上颌前牙宽度比例、微笑位上唇线高度和上颌前牙临床牙冠倾斜角度三个方面差异有统计学意义,因此在前牙美学修复过程中应该加强医患沟通,结合患者心理和生理条件,以获取医患双方都能接受的最佳个性修复效果。Objective To compare different standpoints towards the anterior teeth aesthetics among people in different generation,and set up a reference for the Esthetic restoration. Methods A electronic questionnaire with pictures and visual analog scale,which includes some variable common parameters for anterior teeth aesthetics,such as the width proportion of upper anterior teeth,the width/length proportion of upper anterior teeth,the upper lip line position in smile,the buccal space size and the angle of inclination of clinical crown of upper incisors,was designed in this research. Results The results indicate a significant difference between the dentists′ and layman′s standpoints in upper anterior teeth width proportions（P〈0.05）,upper lip line position in smile（P〈0.05）and the angles of inclination of clinical crown of upper anterior teeth（P〈0.05）in contrast to the size of buccal space（P=0.176）and the upper incisors width/length proportion（P=0.192）. However,no significantly statistical differences were found among different academic careers,income,living place,official rank or different working lifetime in layman′and dentists′standpoints toward the anterior teeth aesthetics（P＆gt;0.05）. Conclusions Significant differences were concluded between dentists and layman′s standpoint toward such common parameters of the anterior teeth aesthetics as the standpoints in upper anterior teeth width proportions,upper lip line position in smile,the angles of inclination of clinical crown of upper anterior teeth. Therefore,we should improve communication between doctors and patients,and obtain the best results of the anterior dental restoration by carefully scrutinizing differences in the above three aspects of the aesthetic standpoints.
