Parks切口在斜视显微手术治疗中的应用

摘　　要：目的观察Parks切口在斜视显微手术治疗中的应用效果。方法 54例斜视住院手术患者作为实验组(采用Parks切口入路治疗),同期20例斜视住院手术患者作为对照组(行传统手术治疗)。对比两组对术后疼痛反应、结膜恢复状况、眼位情况。结果术后第1天疼痛情况,实验组轻度49例(90.7%),中度3例(5.6%),重度2例(3.7%)。对照组轻度13例(65.0%),中度5例(25.0%),重度2例(10.0%)。实验组疼痛情况优于对照组(P<0.05)。术后第1天实验组52例(96.3%)结膜复位良好,无明显伤口痕迹。眼位满意者,近期47例(87.0%),远期46例(85.2%)。结论斜视手术入路Parks切口不损伤角膜缘干细胞,术野暴露充分,术后瘢痕小,结膜与肌肉无粘连,切口小可不缝合,手术方法易掌握,能有效降低疼痛,防止肌肉粘连,减少术后并发症,保证眼部的美观,值得推广。Objective To observe application effect by Parks incision in microsurgical strabotomy. Methods There were 54 strabismus patients in hospital as experimental group（received Parks incision approach for treatment） and another 20 strabismus patients as control group（received conventional surgery）. Comparison was made on postoperative pain reaction, conjunctival recovery condition and ocular position between the two groups. Results In the first day after operation, the experimental group had 49 mild pain cases（90.7%）, 3 moderate cases（5.6%） and 2 severe cases（3.7%）. The control group had 13 mild cases（65.0%）, 5 moderate cases（25.0%）, and 2 severe cases（10.0%）. The experimental group had better pain condition than the control group（P〈0.05）. There were 52 cases（96.3%） in the experimental group with good conjunctival restoration and no obvious scar in 1 d after operation. Among patients with satisfactory ocular position, there were 47 short-term cure cases（87.0%） and 46 long-term cure cases（85.2%）. Conclusion Parks incision approach in strabotomy provides no trauma to limbal stem cells, with fully exposed operative field, small postoperative scar, no conjunctiva and muscle adhesion and non-suture small incision. This method is easily operational to reduce pain, prevent muscle adhesion and decrease postoperative complication. It guarantees good eye appearance and is worth promoting.
