摘 要:目的研究程序性死亡分子1(programmed death-1,PD-1)和程序性死亡配体1(Programmed death ligand 1,PD-L1)在人子宫颈癌中的表达情况及其与临床病例特征的关系。方法选取该院病理科2012年4月-2014年11月份存档的蜡块106例。采用免疫组织化学SP法检测PD-1和PD-L1在48例宫颈癌组织,40例宫颈CIN(宫颈上皮内瘤样病变)组织,18例正常宫颈组织中的表达,并分析其与患者各临床病理特征的关系。结果 PD-1主要表达于肿瘤周围浸润的淋巴细胞上,PDL1主要表达于宫颈癌细胞及癌间质淋巴细胞上。PD-1在宫颈癌组织中的阳性表达率(75%)明显高于宫颈CIN(25%)和正常宫颈组织(0%),差异具有统计学意义(P<0.05);PD-L1的阳性表达率在宫颈癌组织(81.3%)和宫颈CIN组织(79.2%)中差异无统计学意义(P>0.05),但均明显高于正常宫颈组织(0%),差异具有统计学意义(P<0.05)。宫颈癌组织中PD-1、PD-L1的表达均与组织分化程度和FIGO分期相关(P<0.05),与患者年龄、病灶大小及HPV感染无关(P>0.05);宫颈癌组织中PD-1和PD-L1的表达无明显相关性(P>0.05)。结论人子宫颈癌组织中PD-1、PD-L1表达升高,且与肿瘤的发生、发展密切相关。抑制PD-1和/或PD-L1分子有可能成为子宫颈癌免疫治疗的新策略。Objective To research the expressions of programmed death-1( PD-1) and programmed death ligand 1( PD-L1) in cervical squamous cell carcinoma and the relationships with clinicopathological characteristics. Methods A total of 106 paraffin- embedded cervical tissue specimens were selected from Department of Pathology in the hospital from April 2012 to November 2014. Immunohistochemical method was used to detect the expressions of PD- 1 and PD- L1 in 48 cervical cancer specimens,40 cervical intraepithelial neoplasia( CIN) specimens, and 18 normal cervical specimens. The relationships with clinicopathological characteristics were analyzed.Results PD-1 mainly expressed in lymphocytes infiltrating in the tumor,and PD-L1 mainly expressed in cervical cancer cells and interstitial lymphocytes of cancer. The positive expression rate of PD-1 in cervical cancer tissues( 75%) was statistically significantly higher than those in CIN tissues( 25%) and normal cervical tissues( 0%)( P〈0. 05). There was no statistically significant difference in the positive expression rate of PD-L1 between cervical cancer tissues( 81. 3%) and CIN tissues( 79. 2%)( P〉0. 05),but the positive expression rates of PD-L1 in cervical cancer tissues and CIN tissues were statistically significantly higher than that in normal cervical tissues( 0%)( P〈0. 05). The expressions of PD-L1 and PD-1 in cervical cancer tissues were correlated with degree of differentiation and FIGO staging( P〈0. 05),which were not correlated with age,lesion size and HPV infection( P〈0. 05). There was no significant correlation between PD-L1 expression and PD-1 expression in cervical cancer tissues( P〉0. 05). Conclusion The expressions of PD-L1 and PD-1 in human cervical cancer tissues increase,which are closely related to oncogenesis and development of cervical cancer. Inhibiting PD- 1 and / or PD- L1 may become a new strategy in immunotherapy of cervical cancer.
