摘　　要：目的探讨人乳头瘤病毒(HPV)E6/E7 mRNA表达在阴道镜检查阴性女性宫颈上皮内瘤变(CIN)随访评估中的价值。方法选取2014—2015年阴道镜活检阴性(宫颈炎性疾病)且伴有高危型HPV感染、年龄>25岁的195例女性为研究对象,行细胞学、HPV-DNA、E6/E7 mRNA检测。随访1年后,分析E6/E7 mRNA表达与高危型HPV阳性检出率、CINⅡ+发生率的关系。结果随访1年后,E6/E7 mRNA表达阳性者高危型HPV阳性检出率、CINⅡ+发生率率均明显高于E6/E7 mRNA表达阴性者(均P<0.05)。E6/E7 mRNA阳性表达且年龄≥40岁的女性,其CINⅡ+发生率明显高于<40岁者。结论 E6/E7 mRNA阳性表达对CINⅡ+的发生具有一定的预测价值,特别是年龄≥40岁的女性。Objective To assess the value of E6/E7 mRNA expression in follow-up of women negative cervical biopsy. Methods One hundred and ninety-five women aged over 25 with HR-HPV infection and negative colposcopic cervical biopsy underwent cytology, HPV-DNA, E6/E7 mRNA tests from 2014 to 2015. All subjects were followed-up for one year. Results After one year follow-up the women with positive E6/E7 mRNA expression had a higher rate of HR-HPV persistent infection and a higher incidence rate of high grade cervical intraepithelial neoplasia （CINII＋） than those with negative E6/E7 mRNA expression （P〈0.05）, and women aged over 40 with E6/E7 mRNA positive expression had a higher incidence rate of CINIb than those younger than 40. Conclusion Positive expression of E6/E7 mRNA may have a predictive value for CINIb in women with HR-HPV infection and negative cervical biopsy, especially for those aged over 40.
