头痛门诊中军人头痛的临床特征分析

摘　　要：目的 分析在解放军总医院头痛门诊就诊的军人头痛的临床特征。方法 对2011年10月—2012年12月以头痛为主诉来解放军总医院头痛门诊就诊的军人患者进行头痛调查问卷,记录其临床资料和诊治,诊断标准参照国际头痛诊断标准Ⅱ。结果 头痛门诊就诊的军人患者共计85例（男60例）,平均年龄34.16岁（19~69岁）,所有患者中87.1%（74/85）诊断为原发性头痛,包括紧张性头痛34.9%（30例）,偏头痛34.1%（29例）,丛集性头痛14.1%（12例）,其他原发性头痛3.5%（3例）。2.4%（2/85）诊断为继发性头痛,9.4%（8/85）为脑神经痛和中枢引起的面神经痛,为不明的头痛1.2%（1/85）。偏头痛与紧张性头痛的VAS分值比较,二者有统计学差异（P〈0.05）。结论偏头痛与紧张性头痛是军人至头痛门诊就诊最常见的。军人患者的头痛特征既有与普通人群相符合的许多特点,又因其特殊性表现出军队自有的特色。Objective To analyze the clinical features of headache of outpatients armymen visited headache clinic. Methods The study was conducted from Oct. 2011 to Nov. 2012 on the patients with headache from Chinese PLA general hospital as the chief compliaint in Headache Center. A Headache questionnaire was used to collect medical records. Diagnosis of headache was taken according to the International Classification standard Ⅱ of Headache Disorders i.e ICHD-lI criteria. Results A total of 85 armymen visited author＇s Headache Clinic with the median age of 34.16 （19-69） years and the male/female ratio of 2.4/1. The primary headaches （87.1% ） were classified as the following： migraine （34.1% ）, tension-typeheadache （TTH 35.3% ）, trigeminal autonomic cephalalgias （14.1%） and other（3.5%）. Among the rest patients,2.4% were secondary headaches,9.4% were cranial neuralgias and 1.2% were unspecified or not elsewhere classified. The visual analogue scale（VAS） score of migraine was significantly higher than that of TTH （7.52±1.62 vs 4.73±1.60,P〈0.05）. Conclusion Migraine and tension-type headache are the two most common diaguosises for armymen visited. The armymen not only have headache characteristics as well as the general population,but also have many of the characteristics of the military＇s own characteristics because of their special nature of their performance.
