摘  要:目的研究不同途径移植骨髓间充质干细胞(MSCs)治疗D-氨基半乳糖(D—gal)诱导的小型猪急性肝功能衰竭的疗效和相关机制。方法采用D-氨基半乳糖胺诱导猪急性肝功能衰竭模型,比较外周静脉、门静脉、肝动脉和直接肝内注射MSCs治疗猪急性肝功能衰竭的疗效。观察并记录存活情况;术前、术后1、3、5天抽血检测肝功能;肝脏组织HE染色,观察病理情况并进行组织学评分;TUNEL法检测肝细胞凋亡;蛋白印迹检测caspase-3、survivin、AKT及ERK表达水平。结果外周静脉、门静脉、肝动脉、肝内注射MSCs治疗急性肝功能衰竭猪的平均存活时间分别为(4.7±1.5、10.0±2.0、6.0±1.0、3.3±0.6)d,病理学评分分别为9.5±O.5、3.2±1.0、8.2±0.8、10.7±0.6。与D—gal对照组相比,门静脉组小型猪存活时间明显延长(P〈0.05);门静脉组(3.17±1.04,P〈0.05)和肝动脉组(8.2±0.8,P〈0.05)组织病理学评分与D—gal对照组(11.50±1.32)相比显著降低(P〈0.05)。门静脉注射MSCs可使直接胆红素水平显著下降。此外,门静脉组[(25.0±3.4)%,P〈0.05)]和肝动脉组[(40.5±1.0)%,P〈0.05]肝细胞凋亡率与D—gal组[(70.6±8.5)%]比较有统计学意义。门静脉MSCs移植能够降低活化caspase-3水平,提高survivin、AKT及ERK表达水平。结论门静脉注射MSCs可显著改善急性肝衰竭小型猪的肝功能和生存时间,并且促进肝细胞再生和抑制肝细胞凋亡。Objective To explore the most effective route of mesenchymal stem cell transplantation (MSCs) in D-galactosamine (D-gal) induced porcine model with acute liver failure (ALF) and the potential mechanism. Methods BA-MA mini-pigs with D-gal-induced ALF were transplanted with porcine mesen- chymal stem cells (MSCs) through four routes: intraportal injection ( InP), peripheral intravenous injection (PV), hepatic intra-arterial injection (AH) and intrahepatic injection (IH). The survival time was recor- ded. The blood samples before and after MSC transplantation were collected for detecting liver function. Li- ver histology was interpreted and scored. Hepatic apoptosis and regeneration were detected by Terminal De- oxynueleotidyl Transferase dUTP Nick End Labeling (TUNEL) assay and Ki-67 assay. The protein expres- sion of cleaved caspase-3, survivin, AKT and ERK were analyzed by Western blot. Results The average survival time in each group was ( 10.7 ± 1.6) days (InP), (6.0 ± 0.9 ) days (AH), (4.7 ± 1.4) days (PV) , (4. 3 ± 0.8 ) days (IH) when compared with D-gal group [ (3.8 ± 0.8) days ]. The histopathologi- cal scores revealed a significantly decrease in InP group (3.17 ± 1.04, P 〈 0.05 ) and AH group (8.17 ± 0.76, P 〈 0. 05) when compared with that in D-gal group ( 11.50 ± 1.32). The apoptosis rate in InP group ( 25.0 ± 3.4%, P 〈 0.05 ) and AH group (40.5 ± 1.0%, P 〈 0.05 ) was lower than that in D-gal group (70.6 ± 8.5% ). The expression of active caspase-3 was inhibited, while the expression of survivin, AKT and ERK was elevated in InP group. Conclusions The intraportal injection was superior to other pathways for MSCs transplantation. Intraportal MSC transplantation could improve liver function, inhibit cell apopto- sis, promote cell proliferation and prolong the survival in porcine ALF model.
