TAFRO syndrome: A Case Report and Literature Review

摘　　要：目的提高对TAFRO综合征的认识.方法：报道1例TAFRO综合征的临床特征、实验室检查指标、诊治经过及转归,并进行相关文献复习和讨论.结果：女,68岁,因头晕乏力20余年,加重伴胸闷气喘20余天入院.患者有发热、脾脏肿大、双下肢水肿、多浆膜腔积液贫血、血小板减少;骨髓活检病理显示间质网状纤维组织增生显著,淋巴浆细胞聚集.经糖皮质激素及静脉用丙种球蛋白免疫调节治疗、输注红细胞、血小板支持治疗后,患者体温恢复正常、浆膜腔积液吸收、血象恢复良好.结论：TAFRO综合征是一组临床症状的组合,准确诊断有赖于综合临床表现、实验室检查及病理学特征,糖皮质激素治疗为首选治疗,配合积极支持治疗,预后显著改善,临床上需提高对该疾病的认识.Objective To deepen the understanding of c li n i c a l manifestation and treatment patients withTAFRO syndrome.Methods The c li n i c a l data of a patient were analyzed and related literatu res were reviewed.Results A 68-year-old female presented with fever, splemegaly, anasarca. Blood examination showed anemia andthrombocytopenia. Bone marrow biopsy showed a hypercellular marrow with lymphoplasmtoidcellsclustering, andr e t ic u lin f i b r o s i s . After adminstering with glu co co rtico id s, high dose IVIG and best supportive care, thec l in ic a l symptoms vanished.Conclusion Symptoms of patients with TAFRO syndrome are variable.The diagnosis re lie son history of disaese and pathologic examination. Currently, Glucocorticoids can be used as frontline therapy.Immunosuppresive therapy is a promising method. Cases of TAFRO syndrome should be thoroughly investigated ford ifferen tia tin g other diseases.
