摘 要:目的探讨案例教学法(case-based learning,CBL)结合高仿真电脑模拟人在肺部体格检查教学中应用的有效性。方法将在某医院实习的2012级七年制学生(60人)和2012级五年制学生(30人)共90人为研究对象,随机分为实验组45人和对照组45人,均由3名内科脱产带教教师进行肺部体格检查教学,实验组采用CBL结合高仿真电脑模拟人教学方法,对照组采用传统教学方法。两组学生进行统一的肺部查体SP考试和肺部异常体征的考核和问卷调查。结果⑴实验组在肺部查体SP考试成绩和查体手法的规范程度、肺部异常体征考核明显高于对照组,有统计学差异(t值=2.784,P=0.034;t值=2.018,P=0.042;t值=5.385,P=0.016);⑵问卷调查显示实验组在学习兴趣、理论联系实际能力、医患沟通能力、临床思维能力等方面较对照组明显升高(t值=2.186,P=0.037;t值=2.138,P=0.039;t值=2.086,P=0.042;t值=3.038,P=0.019),差异有统计学意义。结论在肺部体格检查教学中应用CBL结合高仿真电脑模拟人教学方法能明显地提高教学质量,有助于医学生熟练掌握肺部体格检查的手法和内容。Objective To investigate the case teaching method(case-based learning,CBL) with high effectiveness simulation computer simulation was applied in the teaching of physical examination of the lungs.Methods Among of Medical Sciences practice grade 2012 of 7-years of students(60) and the class of grade 2012 of 5-years students(30) of a total of 90 human subjects in Xuanwu Hospital of Capital University,randomly assigned as the experimental group 45 and 45 in the control group,by three Department of full-time teachers in teaching of physical examination of the lungs,experimental group by CBL with high simulation computer simulation teaching method,and the control group used the traditional teaching method.Two groups of students to carry out a unified examination of pulmonary SP examination、abnormal signs of the lungs and questionnaire investigation.Results 1Experimental group in the lungs check body SP test result and check method of standardization degree,assessment of lung abnormal signs was significantly higher than that of the control group,there was statistically significant difference(t =2.784,P =0.034;t =2.018,P =0.042;t =5.385,P =0.016);2The questionnaire investigation shows that the experimental group is significantly higher than the control group in the aspects of learning interest,the theory of practical ability,the doctor-patient communication skills,clinical thinking ability and so on(t=2.186,P = 0.037;t =2.138,P =0.039;t =2.086,P =0.042;t =3.038,P =0.019),the difference has statistical significance.Conclusion In the teaching of physical examination of the lungs by CBL node high simulation computer simulation teaching method can obviously improve the quality of teaching,help medical students to master the methods of physical examination of the lungs and content.
