老年乳腺癌临床病理特征及手术方案选择分析

摘　　要：回顾性分析老年乳腺癌的临床病理特征及手术方案的选择。筛选出2012年1月-2014年12月河北医科大学第四医院乳腺中心经病理证实的女性原发性乳腺癌共4 609例,老年乳腺癌（≥65岁）共795例。按年龄65岁-70岁及≥70岁分组,比较两组患者临床病理特点及手术方式。结果显示65岁-70岁组及≥70岁组患者分别554例（12.01%）及241例（5.60%）,初诊Ⅳ期患者及骨转移者≥70岁组均高于65岁-70岁组。两组患者肿瘤学分级差异无统计学意义,但两组患者中Ⅰ级和Ⅱ级者均较高。65岁-70岁组患者选择改良根治术高于≥70岁组患者（χ-2=4.792,P=0.029）,局部切除术以及放弃手术治疗患者均低于≥70岁组。而在乳房切除＋前哨淋巴结活检术、乳房单纯切除术以及乳房切除＋低位淋巴结摘除术方案上两组差异无统计学意义。老年乳腺癌患者自查意识差,但其恶性程度低,由于多伴有其他系统疾病,治疗方案需个体化,手术方式可选择局部扩大切除术,提高患者生活质量。To analyze the clinicopathological features and surgery ways of breast cancer in elderly patients.4 609 female patients of primary breast cancer which were diagnosed by pathology in the 4th Hospital of Hebei Medical University from Jan 2012 to Dec 2014 were analyzed.795 cases are elderly patients（≥65 years old）.All the patients were divided into 65-70 years old group and ≥70years old group.The clinicopathological features and surgeon ways between the two groups compared.Of the 4 609 patients,12.01% was 65-70 years old group,and 5.60% was≥70 years old group.In the≥70 years old group,patients with stage Ⅳ and with bone metastases for first diagnosis were higher than the patients in the 65-70 years old group.There were no statistical significant difference of histological grade between the two groups;however,the gradeⅠ and Ⅱin the two groups were higher.who choose modified radical groups in 65 - 70 years old patients were higher than the patients in ≥70 years old group.The ways of partial resection and patients who give up surgery in 65-70 years old patients were lower than the patients in ≥70 years old group.There had no statistical significant difference in choose the mastectomy＋sentinel lymph node biopsy,mastectomy,breast resection＋low lymph node excision.The self-exams consciousness of elderly patients with breast cancer is poor.But the breast cancer in elderly patients had low degree of malignant.Treatment way should be individualized because of with other system diseases.We can choose local expanded resection operation way to improve the patients quality of life.
