肠梗阻导管治疗结直肠癌根治术后早期低位小肠梗阻综合征的临床疗效

摘　　要：探讨肠梗阻导管治疗结直肠癌根治术后早期低位小肠梗阻综合征的临床疗效。选择结直肠癌根治术后早期低位小肠梗阻综合征患者28例,随机分成采用肠梗阻导管的研究组(n=11例)和采用鼻胃管的对照组(n=17例)。观察两组患者腹部胀痛改善程度、肠梗阻缓解时间、腹部立位片气液平面消失时间、腹围消减程度。所有患者均置管成功。研究组早期低位肠梗阻缓解率100%(11/11)。研究组腹部胀痛改善程度、肠梗阻缓解时间、腹部立位片气液平面消失时间、腹围消减程度均明显优于对照组,其差异有统计学意义(P<0.05)。研究组随访6个月无一例低位肠梗阻综合征复发。肠梗阻导管治疗结直肠癌根治术后早期低位小肠梗阻综合征疗效确切,能够显著改善患者临床表现,值得临床推广。To investigate the clinical application of silicon ileus tube in the therapy of early distal intestinal obstruction syndrome after radical surgery for colorectal carcinoma.Of 28 patients with early distal intestinal obstruction syndrome,who underwent radical surgery for colorectal carcinoma,the patients were randomly divided into silicon ileus tube of study group（n=11）and nasogastiric tube of control group（n=17）.After the procedure,the patients were kept under close observation,focusing on the abdominal distention,the intestinal obstruction reduce time,the vanishing time of intestinal air-liquid plane on abdominal X-ray,and the degree of narrowing of the abdominal circumference,were calculated.The results were compared between the two groups.The tube placement operation was successfully performed in all patients.The study group of early distal intestinal obstruction syndrome of reduce rate was 100%（11/11）.The study group had a significantly alleviate abdominal distention,shorter intestinal reduce time,shorter time of intestinal air-liquid plane on abdominal X-ray,and shrunken abdominal circumference（P〈0.05）.The study group which was followed up 6 months after therapy,no case relapsed of distal intestinal obstruction syndrome.For the therapy of early distal intestinal obstruction syndrome,the placement of silicon ileus tube is effective and reliable.This technique can strikingly improve the clinical manifestations,and its spreading clinical.
