牙体牙髓病科1500例患者与医生关系的初步分析

摘　　要：目的 通过对1500例已治患者与医生关系的回顾性调查,了解牙体牙髓病科医患纠纷的特点。方法 1名主任医师一段时间诊治的全部1500例患者台账资料被录入Exell数据库,将3类医患关系（好、中、差）与相关因素进行卡方检验,并对“差”的病例进行了分析。结果 1 500例患者中,医患关系“好、中、差”分别为4.13%、94.14%和1.73%。医患关系与性别、费用支付方式无关（P〉0.05）,但与年龄、DF、治疗方法、纤维桩或CAD/CAM等修复治疗有关。医患关系“差”的26例中,女性多（21例）,根管治疗的多（21例）,6例投诉,投诉者的处理皆涉及经济赔偿。结论 牙体牙髓病科患者中,女性、41~65岁、RCT者出现医患纠纷的可能性较大。Objective To retrospectively investigate 1500 cases of DoctorPatient Relationship （ DPR） , and understand the characteristics of medical disputes of dental pulp disease. Methods The ledger data of 1500 patients treated by a chief physician for a period of time were put into Exell database and were divided into three kinds of DPR （ satisfaction, normal and dispute） ,then the related factors of chi square test, also the “dispute” of the cases were analyzed. Results In 1500 patients, the satisfaction,normal and dispute were 4.13%, 94.14% and 1.73% respectively. Chi square test results indicated that DPR had nothing to do with gender and the way of payment （ P〉0.05） , but related to age, DF（ Dental Fear） , treatment methods and other repair therapy such as fiber pile or CAD/CAM（Computer Aided Design/Computer Aided Manufacture）. Women （21 cases） were in the majority of the disputes（26 cases）, so were RCT（ Root Canal Treatment） patients （ 21 cases） . 6 cases of complaints, which were involved in the handling of economic compensation. Conclusion Among the dental pulp disease patients, the 41-65 yearold female patients who need RCT have greater likelihood of medical dispute.
