软核白内障行超声乳化手术的技巧

摘　　要：目的探讨软核白内障行超声乳化手术的技巧及处理策略。方法对182例（205眼）软核白内障采用拦截劈核、翻转翘核劈核及浅埋核劈核技术施行超声乳化手术。结果采用拦截劈核53眼（25．85％），翻转翘核劈核62眼（30．25％），浅埋核劈核技术90眼（43．90％）。术后1个月矫正视力≥0．5者186眼（90．73％），≥0．8者126眼（61．46％）。术后角膜水肿25眼（12．2％），均在术后1～3d消退。术中无一眼发生后囊破裂、悬韧带断裂及虹膜损伤。结论软核白内障行超声乳化手术中掌握拦截劈核，翻转翘核劈核，浅埋核劈核技术以及其灵活应用是一种安全有效的手术模式。Objective To investigate the surgical skills and strategies of phacoemulsification in treating soft nucleus cataract. Methods Total of 205 eyes of 182 cases of soft nucleus cataract were treated by three different phacoemulsification techniques which were stop-and-chop, flip-and-chop, or shallow- impale phaco chop. Results There were 205 eyes operated with above mentioned surgical techniques. Of 205 eyes, 53 eyes （25.55%） were treated with stop-and-chop, 62 eyes with flip-and-chop and 90 eyes （43.90%） with shallow-impale phaeo chop technique. The BCVA recovered to more than 0.5 in 186 eyes （90.73%） , and more than 0.8 in 126 eyes （61.46%）. Corneal edema occurred in 25 eyes （ 12.20% ） and disappeared in 1-3 days after operation. No case of suspensory ligament broken, posterior capsular rupture or iris injury emerged. Conclusion These three techniques are stop-and-chop, flip-and-chop and shallow-impale phaco chop, which are safe and efficient in soft nucleus cataract surgery.
