左侧单肺通气条件下经皮经肝射频消融治疗肝顶部癌的疗效和安全性

摘　　要：目的:研究肝顶部癌(LCHD)患者在左侧单肺通气条件下经皮经肝射频消融(PRFA)治疗的临床效果及安全性。方法:临床纳入90例某院2012年3月~2015年3月期间收治的肝顶部癌患者,所有患者按随机数字表法分为两组各45例。其中45例患者在左侧单肺通气条件下采用PRFA治疗作为观察组,另45例患者仅采用PRFA治疗作为对照组。结果:观察组、对照组总有效率分别为82.22%、68.89%,观察组明显较高,P<0.05。观察组治疗时间、穿刺次数少于对照组,P<0.05;而两组住院时间以及治疗费用均无差异,P>0.05。观察组、对照组并发症分别为20%、44.44%,观察组明显较低,P<0.05。观察组患者6个月、12个月以及24个月生存率分别为97.78%、93.33%以及77.78%,明显高于对照组的91.11%、84.44%以及64.44%,P<0.05。结论:LCHD患者在左侧单肺通气条件下行PRFA治疗效果显著,并发症明显减少,生存率提高,值得临床应用及推广。Objective： To study efficacy and safety of treating liver cancer by percutaneous liver radio^re- quency ablation under the condition of ventilation of One-lung on the left side. Methods： 90 cases with liver cancer treated in a hospital from March 2012 to March 2015 were taken as research objects, who were divided into two groups （n=45） according to random number table method. 45 cases were treated by PRFA therapy under the condition of the left single lung ventilation as observation group and the other 45 patients were treated with PRFA therapy only as control group. Results： Total effective rate of observation group and control group were 82.22%, 68.89% respectively, with statistical difference （P〈0.05）; treatment time, punc- ture times of the obervation group were less than those of the control group （P〈0.05） length of hospital stay and cost of treatment of the two groups had no differenee（P〈0.05） complications of the observation group and control group were 20%, 44.44% respectively, with statistical difference（P〈0.05） ; 6 months, 12 months and 24 months survival rates of the two groups were 97.78%, 93.33% and 77.78%, significantly higher than those of the control group 91.11%, 84.44% and 64.44% （P〈0.05）. Conclusion. Efficacy of trea- ting liver cancer by percutaneous liver radiofrequency ablation under the condition of ventilatiofi of one-lung on the left side is remarkable, with high survival rate, worthy of clinical application and promotion.
